LIABILITY WAIVER AND RELEASE FORM
Mamas of Santa Barbara
By signing this document, I, the undersigned, acknowledge and agree to the following terms and conditions in connection with my participation and my child’s participation in Mamas of Santa Barbara classes.
1. Assumption of Risk
I understand that Pilates, including Mommy and Me Pilates, involves physical activity that can be strenuous and may carry inherent risks, including but not limited to the risk of injury. I acknowledge that both myself and my child are participating in the Program voluntarily, and I assume full responsibility for any risks, injuries, or damages that may occur as a result of participation.
2. Medical Clearance
I affirm that I and my child are in good health and physically capable of participating in the Pilates classes. I have consulted a physician or healthcare provider and received medical clearance for physical activity, if necessary. If I or my child experience any discomfort, pain, or health-related issues during the Program, I will immediately inform the instructor and cease participation if needed.
3. Release of Liability
I hereby release, discharge, and hold harmless the Pilates studio, its instructors, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action that may arise out of or in connection with my or my child’s participation in the Program, including any injuries or damages sustained during or after the class.
4. Parental Consent
As the parent or legal guardian of the child named below, I consent to my child’s participation in the Program and agree to be fully responsible for my child’s actions and well-being while participating. I also understand that the instructors are not responsible for my child’s behavior or actions during the class.
5. Photographic and Video Release
I grant permission for the Pilates studio to take photographs or videos of myself and my child during class for promotional purposes. I understand that these images or videos may be used in marketing materials, on the website, or on social media platforms. If I do not wish for my or my child’s likeness to be used, I will inform the instructor in writing prior to the start of the class.
6. Emergency Medical Treatment
In the event of an emergency, I authorize the instructor or other representatives of Mamas of Santa Barbara to seek emergency medical treatment for myself or my child, as deemed necessary. I agree to be responsible for any medical expenses incurred in the event of an injury.
7. Indemnification
I agree to indemnify and hold harmless the Pilates class and its instructors, employees, agents, and affiliates from any and all claims, damages, or liabilities arising from any injury, loss, or damage caused by my actions or my child’s actions during participation in the Program.
8. Agreement to Follow Instructions
I agree to follow all instructions provided by the instructor during the Pilates class to ensure my safety and the safety of my child. I understand that failure to follow the instructor’s guidance may increase the risk of injury.
By signing below, I acknowledge that I have read and fully understand this Liability Waiver and Release Form. I consent to my own and my child’s participation in the Mommy and Me Pilates Program and voluntarily assume all associated risks.
