MOTHER PILATES – LIABILITY WAIVER & RELEASE
By participating in any class, event, or experience hosted by MOTHER PILATES LLC (“MOTHER”), including but not limited to studio classes, outdoor classes, pop-ups, and events held at third-party locations (collectively, the “Activities”), I acknowledge and agree to the following:
I understand that Pilates, fitness activities, babywearing, and participation during pregnancy or postpartum recovery involve inherent risks, including but not limited to injury, illness, physical strain, falls, or harm to myself and/or my child. I voluntarily assume all risks associated with participation.
I confirm that I am physically able to participate and have consulted with a healthcare provider if necessary. I understand that I am responsible for monitoring my own physical condition throughout the Activities.
If attending with a child, I acknowledge that I am solely responsible for my child’s supervision, safety, behavior, and well-being at all times. I understand that MOTHER is not a licensed childcare provider unless explicitly stated otherwise.
I agree to follow all instructions provided by MOTHER instructors and staff and to use all equipment safely and properly. I understand that I may stop participation at any time.
In consideration of being permitted to participate, I hereby release, waive, and discharge MOTHER PILATES LLC, Brittany Carel Breslauer, all instructors, employees, contractors, affiliates, volunteers, host venues, landlords, property owners, sponsors, and event partners from any and all claims, liabilities, demands, damages, costs, or causes of action arising out of or related to participation in the Activities, including those caused by ordinary negligence.
I understand and agree that this waiver applies to myself, my child (if applicable), and my heirs, assigns, and legal representatives.
I grant permission for MOTHER to use photographs and/or video content taken during classes or events for marketing, promotional, and social media purposes unless I provide written notice otherwise.
By agreeing below, I confirm that I have read and understand this waiver and voluntarily agree to its terms.
