Below is a **clear, customizable Zero-Liability Waiver & Release of Claims** you can use for **MOVE4 Mommy & Me Pilates**.

Waiver of Liability, Assumption of Risk & Parental Consent

**Please read carefully before signing. This is a legal document.**

I, the undersigned participant and/or parent or legal guardian of the minor participant(s) listed below, acknowledge and agree as follows:

1. Acknowledgment of Risk
I understand that participation in **MOVE4 Mommy & Me Pilates** classes, sessions, activities, and programs (“Activities”) involves physical movement and exercise that may include, but is not limited to, stretching, strength training, balance work, use of equipment, and interaction between adults and children.
I acknowledge that these Activities carry **inherent risks**, including but not limited to: muscle strain, falls, injury to joints or bones, aggravation of pre-existing conditions, and other physical injuries. I understand that children may act unpredictably and that supervision does not eliminate all risk.

2. Voluntary Participation
I certify that participation is **voluntary** and that I and/or my child are physically and medically able to participate. I understand it is my responsibility to consult a physician regarding any medical conditions, pregnancy, or limitations prior to participation.

3. Assumption of Risk
I knowingly and freely **assume all risks**, known and unknown, foreseeable and unforeseeable, associated with participation in the Activities, for myself and/or my child.

4. Waiver and Release of Liability
To the fullest extent permitted by law, I hereby **waive, release, discharge, and hold harmless** MOVE4 Mommy & Me Pilates, MOVE4, its owners, instructors, contractors, employees, volunteers, agents, and representatives (collectively, the “Released Parties”) from **any and all liability**, claims, demands, actions, or causes of action of any kind arising out of or related to participation in the Activities, including but not limited to claims based on **negligence**.

This waiver applies to **personal injury, illness, property damage, or death**, whether suffered by me or my child.

5. Zero Liability Agreement
I expressly agree that the Released Parties shall bear **no liability whatsoever** for any injury, loss, or damage arising from participation in MOVE4 Mommy & Me Pilates Activities, regardless of cause, to the maximum extent allowed by law.

6. Parental Consent and Indemnification 
If signing on behalf of a minor, I certify that I am the parent or legal guardian of the minor participant(s). I consent to my child’s participation and agree to **indemnify and defend** the Released Parties from any claims brought by or on behalf of the minor.

7. Emergency Medical Consent
In the event of an emergency, I authorize MOVE4 Mommy & Me Pilates to obtain medical treatment for me and/or my child if necessary. I understand I am financially responsible for any medical care provided.

 8. Photo and Video Release (Optional)
I grant permission for photographs or video recordings taken during Activities to be used for promotional or educational purposes without compensation, unless I provide written notice opting out. 

 9. Governing Law
This agreement shall be governed by and construed in accordance with the laws of the State of Florida.

10. Severability
If any portion of this agreement is found to be unenforceable, the remaining portions shall remain in full force and effect.
