ASSUMPTION OF RISK

I understand that participation in Pilates and fitness activities, including “Mommy & Me” classes involving infants or children, involves inherent risks. These risks include, but are not limited to, muscle strain, injury, falls, illness, and risks related to caring for and handling my child during class.

I voluntarily choose to participate in all activities offered by MOVE4 Mommy & Me Pilates and assume full responsibility for any and all risks, injuries, or damages, known or unknown, which may occur to myself or my child as a result of participation.

MEDICAL CLEARANCE

I affirm that I am physically able to participate in Pilates and fitness activities and that my child is healthy and able to be present in a group fitness environment. I understand that it is my responsibility to consult with a physician regarding any medical concerns for myself or my child prior to participation.

RELEASE OF LIABILITY

In consideration of being allowed to participate, I hereby release, waive, discharge, and hold harmless MOVE4 Mommy & Me Pilates, its owners, instructors, employees, volunteers, affiliates, event venues, and sponsors from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me or my child, whether caused by negligence or otherwise.

CHILD RESPONSIBILITY

I understand that I am solely responsible for supervising my child at all times during class. I acknowledge that instructors are not responsible for childcare, supervision, or safety of my child during sessions.

PHOTOGRAPHY & MEDIA RELEASE (Optional)

I grant permission for photographs or videos taken during classes or events to be used for promotional purposes by MOVE4 Mommy & Me Pilates, without compensation.
If I do not grant permission, I will inform the instructor prior to class. 

ACKNOWLEDGMENT & AGREEMENT

I have read and fully understand this waiver and release of liability. I understand that by signing this document, I am giving up legal rights on behalf of myself and my child. I agree to participate voluntarily and acknowledge this waiver is binding.
