1. Acknowledgment of Risk
I acknowledge that participating in a workout or fitness class involves inherent risks, including but not limited to injury, illness, muscle strain, sprains, fractures, heart attacks, and other health conditions. I understand these risks and voluntarily choose to participate, fully assuming all responsibility for any such risks.

2. Health Declaration
I certify that I am in good physical condition and able to participate in this fitness program. I have disclosed any known medical conditions or physical limitations to the instructor prior to participation.

3. Waiver and Release
In consideration for being allowed to participate in the workout class, I hereby release, discharge, and hold harmless the instructor(s), business owner(s), venue, affiliates, employees, agents, volunteers, and all related parties from any and all liability, claims, demands, or causes of action for injuries or damages arising out of or related to my participation.

This waiver includes but is not limited to liability arising from the negligence or fault of the entities or persons released, whether passive or active.

4. Emergency Authorization
In the event of an injury or medical emergency, I authorize the instructor(s) or staff to obtain emergency medical treatment on my behalf and agree to be responsible for any resulting expenses.

5. Photography and Media Release
I grant permission to use photographs, videos, or recordings taken during the class for promotional or marketing purposes without compensation.
