FLX Move Pilates Experience Studios Waiver and Consent
By signing below, I, [FullName], acknowledge and agree to the following:
1. Physical Health: I represent that I am in good health and have consulted a physician regarding my participation in FLX Move Pilates classes, programs, and workshops. I will follow all instructions to ensure safe participation.=
2. Physical Waiver: I hereby waive and release FLX Move Pilates, its owners, employees, and instructors from any claim, demand, or cause of action related to my participation in any classes, workshops, or other activities. I acknowledge and accept all risks, injuries, or damages, known or unknown, that may occur.=
4. Consent for Photography and Recording: I consent to FLX Move Pilates Studio taking photographs or videos of me during activities for use in marketing, social media, and promotional materials.
5. Late Cancellation Policy: I understand that cancellations or changes made less than 12 hours before a scheduled class or less than 24 hours before a personal stretch session will incur a $25 fee per occurrence and loss of the class to fulfill a regular charge.
