Participant Liability Waiver and Release of Claims
Dose of Sculpt – Outdoor Mat Pilates Event
PLEASE READ CAREFULLY BEFORE SIGNING. THIS IS A LEGAL DOCUMENT.
1. Assumption of Risk: I understand that participation in Pilates classes and physical exercise involves inherent risks,
including but not limited to muscle soreness, strains, sprains, falls, dehydration, or other physical injuries. I
acknowledge that I am voluntarily participating and assume all risks associated with participation, whether known or
unknown, including those associated with outdoor conditions such as uneven surfaces, weather, or environmental
factors.
2. Health Declaration: I affirm that I am in good physical condition and do not suffer from any condition that would
prevent or limit my participation. I understand that it is my responsibility to consult with a physician before beginning any
fitness program. I will inform the instructor of any injuries, medical conditions, or changes to my health status prior to
each class.
3. Release of Liability: I voluntarily agree to release, waive, discharge, and hold harmless Dose of Sculpt, its owners,
instructors, employees, volunteers, agents, sponsors, and affiliates from any and all claims, demands, liabilities,
damages, expenses, or causes of action, present or future, arising out of or connected with my participation, including
injury, illness, or death, whether caused by negligence or otherwise.
4. Indemnification: I agree to indemnify and hold harmless Dose of Sculpt and associated parties from any loss,
liability, damage, or costs incurred as a result of my participation.
5. Photography & Media Consent: I give permission for Dose of Sculpt to take photographs or videos of me during
classes/events and to use them for promotional and marketing purposes, without compensation.
6. Cancellation & Event Policies: I acknowledge that I have read and understand the cancellation, refund, and late
arrival policies provided by Dose of Sculpt. I agree to abide by all event rules and follow instructions given by
instructors.
7. Severability: If any portion of this agreement is found to be invalid or unenforceable, the remainder shall remain in
full force and effect.
