Waiver and Release of Liability

I, the undersigned participant, acknowledge that I have voluntarily chosen to participate in a Pilates class taught by Angel Ogunnaike.

I understand that participating in Pilates involves physical movement and exertion that carries a risk of injury, including but not limited to, muscle strain, falls, abnormal blood pressure, fainting, heart disorders, and in rare cases, serious injury or death.

I affirm that I am in good physical condition and do not suffer from any disability or condition that would prevent or limit my participation in this class. I acknowledge that it is my responsibility to consult with a physician prior to and regarding my participation in any fitness or wellness program, including Pilates.

I hereby assume full responsibility for any and all risks, injuries, or damages known or unknown that I might incur as a result of participating in this class.

I knowingly, voluntarily, and expressly waive any claim I may have against Angel Ogunnaike for injuries or damages that I may sustain as a result of participating in this class, whether caused by negligence or otherwise.

I grant Angel Ogunnaike permission to take photographs and/or videos during the class for promotional use on social media, websites, or marketing materials.
