Liability Waiver & Release form for mat pilates class:
*By purchasing this ticket, I understand and agree to the following:
1. Voluntary Participation- I am voluntarily participating in this mat pilates class offered by Maria Rodriguez and Three Birds Cafe.
2. Assumption of Risk- I acknowledge that exercise involves physical movement and carries the possibility of injury, including by not limited to muscle strains, sprains, abnormal blood pressure, fainting, heart disorders or other physical injury. I knowingly and voluntarily assume all such risks when purchasing a ticket and engaging in mat pilates class movements.
3. Medical Clearance- I affirm that I am good physical condition and do not suffer from any condition that would limit my participation. I have consulted with a physician regarding any concerns or medical conditions, including pregnancy, prior to attending this class.
4. Release of Liability- I release and discharge, Maria Rodriguez, the instructor, and any facility/location hosting this class from any and all liability, claims, demands, or causes of action that may arise from my participation, except in cases of gross negligence or willful misconduct.
5. Photography/Media- I consent to the use of photos or videos taken during class for promotional purposes. If I would prefer to not be photographed or videoed, I will let the instructor know before the class starts!
6. Refunds- refunds will be agreed to by case by case basis. 
6. Acknowledgement- By purchasing this ticket, I acknowledge I have read these above statements and waiver, I fully understand the terms and voluntarily agree.
