I voluntarily assume full responsibility for any risks, injuries, or damages that may result from my participation.
By signing below, I agree to release, waive, and discharge Pilates Unplugged and any event affiliates, organizers, vendors, or location hosts from any and all liability, claims, demands, or actions for injury, loss, or damages arising out of or related to participation in this class.
I also understand that photos or video may be taken during the event for promotional purposes, and I grant permission for my image to be used unless I inform the instructor otherwise.
