ASSUMPTION OF RISK AND RELEASE
I, the undersigned participant, acknowledge that I am voluntarily participating in pilates and activities
(the "Activities") organized by Rocio Munoz. I understand that these Activities involve inherent risks, including but
not limited to:
 Risk of injury from physical exercise and movement
 Risk of slip, trip, and fall injuries
 Risk of muscle strain, sprains, or other physical injuries
 Risk of injury from equipment or facility conditions
 Risk of injury from interaction with other participants
I acknowledge that I am participating in these Activities at my own risk and that I have been advised to consult with
a physician before participating in any physical activity.
RELEASE AND WAIVER OF LIABILITY
In consideration for being permitted to participate in the Activities, I hereby:
1. RELEASE AND DISCHARGE the event organizer, property owner, instructors, volunteers, and their
respective heirs, successors, and assigns (collectively, the "Released Parties") from any and all liability,
claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that
may be sustained by me while participating in the Activities.
2. WAIVE AND COVENANT NOT TO SUE the Released Parties for any claims or damages arising from my
participation in the Activities, whether caused by the negligence of the Released Parties or otherwise.
3. INDEMNIFY AND HOLD HARMLESS the Released Parties from any loss or liability incurred as a result of
my participation in the Activities.
MEDICAL CONDITION ACKNOWLEDGMENT
I certify that I am in good physical condition and have no medical conditions that would prevent safe participation
in the Activities. I agree to immediately inform the organizer of any changes to my health status.
PHOTOGRAPHY AND MEDIA RELEASE
I consent to the use of photographs, videos, or other media taken during the event for promotional purposes.
GOVERNING LAW
This waiver shall be governed by the laws of the State of California. If any provision of this waiver is found to be
unenforceable, the remaining provisions shall remain in full force and eƯect.
ACKNOWLEDGMENT
I have read this waiver carefully and understand its contents. I am aware that this is a release of liability and I sign it
voluntarily.
