POP DIVAS DANCE CLASS AT SUNSET
PARTICIPATION WAIVER & RELEASE OF LIABILITY

By participating in Pop Divas Dance Class at Sunset, I acknowledge and agree that I am physically able and medically fit to participate in the dance, exercise, movement, and other physical activities included in the event.

I understand that participation in dance and physical activity involves inherent risks, including but not limited to injury, illness, dizziness, falls, strains, sprains, or other physical harm. I voluntarily choose to participate and assume all risks associated with my participation.

I agree to release and hold harmless the event organizers, Sip & Shine LLC, Herbalife, and their respective owners, employees, instructors, coaches, representatives, affiliates, and volunteers from any and all liability, claims, damages, or injuries that may arise from or be related to my participation in Pop Divas Dance Class at Sunset, except where prohibited by law.

I understand that I should consult with a physician before participating in physical activity if I have any medical condition, health concern, injury, or doubt about my ability to safely participate.

Please check with your physician before participating if any of the following apply to you:

• You have been prescribed medication for blood pressure or a heart condition.
• A physician has advised you to participate only in specific types of physical activity.
• You experience chest pain during physical activity.
• You have experienced chest pain within the past month while not participating in physical activity.
• You have lost your balance due to dizziness or have experienced unexplained fainting.
• You have a bone, muscle, or joint problem (for example, involving your back, knee, hip, or ankle) that could worsen with physical activity or dance.
• You are pregnant or have given birth within the past six months.
• You have any injury, medical condition, or other health concern that may affect your ability to safely participate.
• There is any other reason why you believe you should not participate in physical activity.

I understand that if I experience dizziness, weakness, lightheadedness, nausea, chest pain, shortness of breath, pain, or any other discomfort during the event, I must stop participating immediately and seek appropriate medical attention.

I understand that participation in Pop Divas Dance Class at Sunset is voluntary and that I am participating entirely at my own risk and responsibility.

PHOTO & VIDEO RELEASE

I grant the event organizers permission to photograph and/or record me during Pop Divas Dance Class at Sunset. I understand that these photographs and videos may be used for promotional, marketing, social media, advertising, and other business purposes by Sip & Shine LLC without additional compensation or prior approval.

By completing and submitting this waiver, I confirm that I have read, understood, and voluntarily agreed to all of the statements and terms contained in this document. I acknowledge that I have had the opportunity to ask questions and that I am choosing to participate in Pop Divas Dance Class at Sunset voluntarily and at my own risk.
