🐕 Optional: Yoga block, ankle or wrist weights, or booty band

PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR RIGHTS.
I, the undersigned, acknowledge that I have voluntarily chosen to participate in a Pilates class or event organized by Pilates Hotties, led by Danna Gadalla and her team. I understand and agree to the following:

Assumption of Risk
I understand that participating in Pilates involves physical exertion and may include exercises that carry a risk of injury. I confirm that I am voluntarily participating and assume all risks associated with this activity, including but not limited to falls, muscle strains, sprains, or other injuries.

Health and Medical Condition
I affirm that I am physically fit and have no medical conditions that would prevent my safe participation. I agree to consult a physician before participating if I have any concerns. Before the class begins, I will notify the instructor of any existing injuries or limitations.

Release of Liability
I hereby release, waive, and discharge Pilates Hotties, Danna Gadalla, instructors, event organizers, sponsors, venue owners, and all affiliates from any and all claims, liabilities, demands, or causes of action related to any injury, illness, loss, or damage I may sustain as a result of my participation.

Photography and Media Release
I grant Pilates Hotties permission to use photos, videos, or recordings of me taken during the event for promotional and marketing purposes without compensation.

Emergency Medical Treatment
In the event of an emergency, I authorize Pilates Hotties representatives to obtain necessary medical treatment for me. I understand that I am responsible for any associated medical costs.

Acknowledgment of Policies
I agree to follow all class guidelines and instructions from instructors to ensure a safe and enjoyable experience for all participants.

By signing below, I acknowledge that I have read, understood, and agreed to the terms of this waiver. I understand that I am giving up substantial rights by signing it and do so voluntarily.
