By checking the acknowledgment box on SweatPals, I confirm that I have read, understand, and voluntarily agree to the following Waiver and Release in consideration for being allowed to participate in the Real REVIVAL | Snow Adventure | Weekend Getaway (“Retreat”) hosted by Natty Performance LLC and Revival Wellness Experience LLC (collectively, the “Hosts”).

1. EVENT DESCRIPTION & GENERAL ACKNOWLEDGMENT

I understand that the Retreat:
	•	Takes place at Village Camp Flagstaff, where lodging will be provided in cabins.
	•	Includes shuttle-style transportation provided by the Hosts between Village Camp and Arizona Snowbowl Ski Resort (“Snowbowl”).
	•	Includes activities, programming, community events, meals, wellness experiences, and group hangouts organized by the Hosts.
	•	Does NOT include ski/snowboard lift tickets, rentals, lessons, or access fees — these are my responsibility and my choice to engage in.
	•	Is not sponsored by, affiliated with, or officially partnered with Village Camp or Arizona Snowbowl. The Retreat is independently created and run by the Hosts.

I understand that participation is voluntary and may include outdoor winter activities, uneven terrain, cold weather, physical exertion, transportation, and social programming.

2. ASSUMPTION OF RISK

I acknowledge that participating in the Retreat involves inherent risks, including but not limited to:

Environmental & Outdoor Risks
	•	Snow, ice, slippery surfaces, extreme temperatures, altitude, weather changes.
	•	Uneven ground, stairs, trails, and cabin surroundings.

Transportation Risks
	•	Riding in vans, shuttles, or vehicles operated by the Hosts or third parties.
	•	Risks of collision, sudden stops, adverse road conditions, and driver error.

Activity Risks
	•	Optional recreational activities, hiking, walking, fitness, wellness sessions.
	•	Independent skiing, snowboarding, tubing, sledding, or snow sports at Snowbowl or elsewhere (which I fully assume responsibility for).

Health & Medical Risks
	•	Illness, altitude sickness, frostbite, hypothermia, dehydration.
	•	Dietary exposure, allergies, food-borne illness.
	•	Exposure to viruses including colds, flu, and COVID-19.

Property & Personal Risks
	•	Loss, theft, or damage to personal items in cabins, shuttles, or public spaces.

I freely and voluntarily assume full responsibility for all risks, known and unknown, inherent or incidental to the Retreat.

3. RELEASE OF LIABILITY

To the fullest extent permitted by Arizona law, I hereby release, waive, and discharge:
	•	Natty Performance LLC
	•	Revival Wellness Experience LLC
	•	Their owners, managers, officers, contractors, volunteers, employees, partners, and agents

(collectively, the “Released Parties”)

from any and all claims, demands, causes of action, damages, losses, injuries, or liabilities of any kind arising from or related to my participation in the Retreat, including claims arising from the ordinary negligence of the Released Parties.

This includes but is not limited to:
	•	Accidents, injuries, illness, or death.
	•	Transportation incidents.
	•	Weather-related or environmental hazards.
	•	Activities undertaken at Snowbowl or during free time.
	•	Lodging-related issues, slip and fall incidents, or cabin conditions.
	•	Lost, stolen, or damaged property.

This release does not waive claims that cannot legally be waived under Arizona law.

4. THIRD-PARTY VENUES (VILLAGE CAMP & SNOWBOWL)

I understand that:
	•	Village Camp Flagstaff and Arizona Snowbowl are independent third-party entities, not owned or controlled by the Hosts.
	•	The Hosts are not responsible for the acts, omissions, or conditions of Village Camp, Snowbowl, or their employees, contractors, or facilities.
	•	Any risks or injuries that occur due to conditions at Village Camp or Snowbowl (including cabins, walkways, slopes, ski areas, or shuttle services) are outside the Hosts’ control.

I agree that any disputes or claims related to Village Camp or Arizona Snowbowl are strictly between me and those respective entities.

5. HEALTH, MEDICAL CARE & INSURANCE

I certify that:
	•	I am in good health and able to safely participate in Retreat activities.
	•	I have no condition that would make participation unsafe.
	•	I will disclose allergies or dietary restrictions to the Hosts in advance.

I understand:
	•	The Hosts do not provide medical evaluations, medical care, or medical personnel.
	•	In a medical emergency, the Hosts may seek treatment on my behalf.
	•	I am solely responsible for all resulting medical costs.
	•	I am responsible for maintaining my own medical insurance.

6. FOOD, DIETARY NEEDS & SUBSTANCES

I understand that food and beverages may be served and that:
	•	It is my responsibility to monitor what I consume.
	•	The Hosts cannot guarantee allergen-free meals or environments.
	•	If alcohol is present, I am solely responsible for my consumption and any related consequences.

I agree not to engage in dangerous activities while impaired.

7. LODGING, PROPERTY, & CABIN USE

I understand:
	•	Cabins may be shared spaces with other participants.
	•	I am responsible for securing my own personal belongings.
	•	The Hosts are not responsible for loss, damage, or theft of my items.
	•	I will treat the lodging and shared spaces respectfully and follow all posted rules.

8. TRANSPORTATION WAIVER

I understand that the Hosts may provide group shuttles or transportation between Village Camp and Snowbowl. I agree that:
	•	Transportation carries inherent risks.
	•	I will follow all safety instructions, including seatbelt use.
	•	The Hosts are not liable for accidents, road conditions, or transportation delays.

9. CODE OF CONDUCT

I agree to:
	•	Behave respectfully and safely.
	•	Follow all instructions from the Hosts, Village Camp staff, and Snowbowl personnel.
	•	Not engage in harassment, unsafe conduct, illegal activity, or disruptive behavior.

The Hosts may remove me from the Retreat without refund if I violate these terms.

10. MEDIA RELEASE

I grant the Hosts full permission to:
	•	Photograph, record, or capture video/audio of me during the Retreat.
	•	Use my image, voice, likeness, or appearance in promotional, marketing, or recap content.
	•	Publish or distribute such media worldwide, in any format, in perpetuity, without compensation.

I waive any right to inspect or approve final materials.

If I prefer not to appear prominently, I understand it is my responsibility to avoid front-row positions or notify the Hosts beforehand.

11. INDEMNIFICATION

I agree to indemnify and hold harmless the Released Parties from any claims, liabilities, or expenses (including legal fees) arising from:
	•	My actions or omissions during the Retreat.
	•	My breach of this waiver.
	•	Injuries or damages I may cause to others.

12. WEATHER, CHANGES, & CANCELLATIONS

I acknowledge that:
	•	Weather in Flagstaff and Snowbowl may cause delays, activity changes, or cancellations.
	•	The Hosts reserve the right to adjust programming for safety or logistical reasons.
	•	Refunds or credits follow the Hosts’ stated cancellation policies and are independent of this waiver.

13. GOVERNING LAW & VALIDITY

This waiver is governed by Arizona law.
If any part is invalid, the remaining sections remain enforceable.
