I acknowledge that participating in a Pilates class and any activations such as cupping/ massage involves physical activity that may carry inherent risks of injury, including but not limited to muscle strain, sprains, and other physical injuries. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in this class.
I affirm that I am in good physical condition and do not suffer from any medical condition that would prevent or limit my participation in this activity. I voluntarily choose to participate in this class with full knowledge of the risks involved.
In consideration of being permitted to participate in this Pilates class, I, on behalf of myself, my heirs, personal representatives, or assigns, do hereby release, waive, and discharge the instructor, venue, organizers, and any affiliates, employees, or volunteers from any and all claims, demands, or causes of action for injuries, losses, or damages, including but not limited to property damage or personal injury, arising out of or connected to my participation in the class, regardless of whether such injury or damage is caused by negligence.
I hereby consent to receive medical treatment deemed necessary in the event of injury, accident, or illness during the class and agree to be responsible for any costs incurred.
I grant permission to the instructor, event organizers, and their authorized representatives to take and use photographs, video recordings, or digital images of me during the Pilates class for promotional, marketing, or educational purposes, in print, online, and on social media. I understand that I will not receive any compensation for the use of such media and waive any right to inspect or approve the final product.
