ASSUMPTION OF RISK
I acknowledge that I am voluntarily participating in a mat Pilates class and related activities at the Reset + Realign event. I understand that physical exercise involves inherent risks, including but not limited to muscle strain, injury, slips, falls, heart complications, equipment failure, or other physical or emotional injury. I knowingly and voluntarily assume full responsibility for any risks, injuries, or damages, whether known or unknown, that may occur as a result of my participation.
HEALTH & MEDICAL ACKNOWLEDGMENT
I certify that I am physically capable of participating and have no medical condition, injury, or limitation that would prevent my safe participation. I understand it is my responsibility to consult a physician prior to participating if needed and to inform the instructor of any limitations.
PREGNANCY DISCLAIMER
If I am pregnant, suspect I may be pregnant, or become pregnant, I acknowledge that I have consulted (or will consult) a qualified healthcare provider prior to participating and voluntarily assume all associated risks.
WAIVER & RELEASE OF LIABILITY
In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless Ariel Velasco, Reset With Ariel, Vows Venue, event organizers, vendors, sponsors, volunteers, employees, contractors, and all other affiliated parties from any and all claims, liabilities, demands, damages, or causes of action arising from my participation, including those resulting from negligence.
I agree not to sue or bring any legal action against any of the released parties. This release shall be binding upon my heirs, representatives, and assigns.
INDEMNIFICATION
I agree to indemnify and hold harmless all released parties from any claims, damages, costs, or expenses (including attorney fees) arising from my participation or conduct during the event.
PROPERTY DAMAGE
I accept responsibility for any damage I cause to the venue, equipment, or property during the event.
PHOTO & VIDEO RELEASE
I understand that photography and video recording will take place during the event. By participating, I grant full permission and irrevocable consent for my image, likeness, voice, and appearance to be captured and used in photographs and videos for promotional, marketing, social media, and educational purposes by the host and affiliated parties, without compensation.
COVID-19 & COMMUNICABLE ILLNESS RELEASE
I acknowledge that participation in group fitness activities may increase the risk of exposure to COVID-19 or other illnesses. I voluntarily assume all such risks and release all parties from related claims.
LEGAL AGREEMENT
I understand this document is a legally binding release. If any portion is found invalid, the remainder shall remain in full force. This agreement shall be governed by the laws of the State of California.
By purchasing a ticket, I acknowledge that I have read, understand, and voluntarily agree to all terms of this waiver.
