Set inside the upscale atmosphere of Carbon Chicago, guests will experience a high-energy day party featuring a live DJ, recovery stations, wellness and hydration vendors, brand giveaways, content creation spaces, product sampling, and community connection with some of the most influential people in Chicago’s fitness and wellness space.

PARTICIPANT WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND MEDIA CONSENT

This Waiver applies to participation in any event, activation, experience, or program organized and operated by LH Fitness Solutions LLC d/b/a Reset Recess Experience ("Reset Recess Experience").

By registering for, attending, or participating in this Event, I acknowledge and agree to the following:

1. ASSUMPTION OF RISK

I understand that Reset Recess Experience is a wellness, recovery, social, and community-based event that may include activities such as recovery services, compression therapy, stretching, mobility work, massage and bodywork services, cold plunge and contrast therapy, wellness demonstrations, product sampling, food and beverage consumption, music, dancing, networking, fitness-related activities, and sponsor or vendor activations.

I understand that participation in these activities involves inherent risks, including but not limited to physical injury, illness, allergic reactions, dehydration, slips, trips, falls, equipment-related incidents, property loss, and other risks associated with wellness, recovery, fitness, and social events.

I voluntarily assume all known and unknown risks associated with my attendance and participation.

2. HEALTH ACKNOWLEDGMENT

I certify that I am physically capable of participating in any activities I choose to engage in during the Event.

I understand that it is my responsibility to consult with a physician or qualified healthcare provider regarding any medical condition, injury, allergy, medication, pregnancy, or health concern that may affect my ability to safely participate.

I acknowledge that LH Fitness Solutions LLC d/b/a Reset Recess Experience, event staff, vendors, sponsors, partners, and venue representatives do not provide medical advice, diagnosis, or treatment.

3. VENDOR AND SPONSOR PARTICIPATION

I understand that independent vendors, sponsors, and service providers may offer products, demonstrations, consultations, recovery services, wellness experiences, or promotional activations during the Event.

My participation with any vendor, sponsor, or service provider is voluntary and undertaken at my own discretion and risk.

I acknowledge that certain vendors may require additional waivers, intake forms, or participation agreements before providing services.

4. RELEASE OF LIABILITY

To the fullest extent permitted by law, I release, waive, and hold harmless LH Fitness Solutions LLC d/b/a Reset Recess Experience, its owners, officers, employees, contractors, volunteers, sponsors, vendors, partners, affiliates, venue owners, venue operators, representatives, and event personnel from any claims, liabilities, damages, losses, costs, or expenses arising from or related to my attendance, participation, use of services, consumption of products, or presence at the Event.

This release includes claims arising from ordinary negligence to the fullest extent permitted by applicable law.

5. PERSONAL RESPONSIBILITY AND CONDUCT

I agree to conduct myself in a safe, respectful, and responsible manner at all times.

I agree to follow all instructions provided by event staff, venue personnel, vendors, security personnel, and service providers.

Reset Recess Experience reserves the right to deny entry or remove any participant whose conduct is unsafe, disruptive, threatening, intoxicated, inappropriate, or otherwise inconsistent with the purpose and environment of the Event.

6. PERSONAL PROPERTY

I understand that I am solely responsible for my personal belongings.

LH Fitness Solutions LLC d/b/a Reset Recess Experience, venue operators, vendors, sponsors, and event personnel shall not be responsible for lost, stolen, damaged, or misplaced property.

7. PHOTO, VIDEO, AND MEDIA RELEASE

I understand that photographs, video recordings, interviews, livestreams, and other media may be captured during the Event.

By attending, I grant LH Fitness Solutions LLC d/b/a Reset Recess Experience, its partners, sponsors, vendors, media representatives, and affiliates permission to use my image, likeness, voice, appearance, and participation in photographs, videos, promotional materials, social media content, advertisements, event recaps, and future marketing efforts without compensation or additional approval.

8. EVENT CHANGES

I understand that event programming, vendors, sponsors, services, activities, schedules, and offerings may change without notice due to operational, safety, weather, venue, or logistical considerations.

9. ACKNOWLEDGMENT

By registering for or attending Reset Recess Experience, I acknowledge that:

• I have read and understand this Waiver and Release of Liability.

• I voluntarily assume the risks associated with participation.

• I agree to the terms outlined above.

• I understand that participation is voluntary and that I may choose not to participate in any activity.

• I understand that this agreement applies to the current Event and any future Reset Recess Experience events or activations in which I choose to participate.

By completing registration and/or attending the Event, I acknowledge and accept these terms.
