Rise Pilates Club meets the Northwestern Beach for a pre-Chicago Stars FC game!&#x20;

We will move through 45 minutes of all levels friendly pilates before coming together to socialize at and enjoy the Chicago Stars FC game at Martin Stadium.&#x20;

Event Opens - 11:00 am

Class Begins - 11:30 AM

Class Concludes - 12:15 PM

Group Walk to Game - 12:45 PM

WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate in fitness classes, training sessions, and/or other activities conducted by Rise Pilates Club, including sessions led by Olivia Santelli, I, the undersigned participant, hereby acknowledge, understand, and agree to the following:

Voluntary Participation
I voluntarily choose to participate in fitness activities offered by Rise Pilates Club and instructor Olivia Santelli. I recognize that physical exercise and Pilates activities require physical exertion and carry a risk of injury, including but not limited to, muscle strain, sprains, falls, heart attacks, and other health complications.

Assumption of Risk
I understand and agree that my participation in any class, workshop, or activity is entirely at my own risk. I represent that I am physically fit to participate and have no medical condition that would prevent my full participation in the fitness program.

Medical Clearance
I affirm that I have either had a physical examination and have been given my physician’s permission to participate or that I have decided to participate without the approval of my physician and do so at my own risk.

Release and Waiver
I hereby release, waive, discharge, and hold harmless Rise Pilates Club, Olivia Santelli, and any of their respective employees, contractors, representatives, or affiliates from any and all liability, claims, demands, damages, or causes of action arising from or related to any injury, illness, or death that may result from my participation in activities associated with Rise Pilates Club, whether caused by negligence or otherwise.

Personal Responsibility
I agree to follow all instructions provided during classes and understand that I am responsible for monitoring my own condition throughout each session. I will immediately inform the instructor if I experience any discomfort or pain during participation.

Media Release 
I grant Rise Pilates Club and Olivia Santelli the right to take photographs or video footage of me during classes and to use these for promotional purposes.

Severability
If any provision of this waiver is held to be invalid or unenforceable, the remaining provisions shall continue to be fully effective.
