SALT / THE EDIT PILATES LIABILITY WAIVER, RELEASE OF CLAIMS, ASSUMPTION OF RISK, AND MEDIA RELEASE

By signing below, I acknowledge that I am voluntarily participating in classes, wellness events, fitness instruction, outdoor programming, beach workouts, and related activities organized, hosted, sponsored, instructed, or operated by The Edit Pilates, SALT, Alexandra Banks, the Chicago Park District, and each of their respective owners, officers, board members, employees, instructors, contractors, volunteers, agents, sponsors, vendors, affiliates, representatives, successors, and assigns (collectively, the “Released Parties”).

I understand that participation in physical activity and outdoor events involves inherent risks, including but not limited to bodily injury, muscle strains, sprains, fractures, falls, dehydration, overexertion, exposure to heat, sun, wind, uneven terrain, equipment-related injuries, contact with other participants, and exposure to communicable illnesses or diseases. I knowingly and voluntarily assume all risks associated with participation, whether known or unknown, foreseeable or unforeseeable.

To the fullest extent permitted under Illinois law, I knowingly and voluntarily waive, release, discharge, and covenant not to sue the Released Parties from any and all claims, liabilities, demands, damages, losses, costs, expenses, or causes of action arising out of or related to my participation in these activities, including claims arising from the ordinary negligence of the Released Parties, including but not limited to negligent instruction, supervision, staffing, event organization, maintenance of equipment, premises conditions, or acts or omissions occurring before, during, or after participation.

This waiver applies to all activities taking place at Chicago Park District property, beaches, parks, studios, outdoor venues, public spaces, and related event locations.

I represent that I am physically able to participate in these activities and do not have any medical condition that would make participation unsafe. I understand that I am solely responsible for monitoring my own physical condition and agree to stop participation immediately if I experience pain, dizziness, illness, or discomfort. I understand that the Released Parties are not medical professionals and do not provide medical advice.

I authorize the Released Parties to obtain emergency medical treatment on my behalf if deemed necessary and agree to be solely responsible for any resulting medical expenses.

I understand that the Released Parties are not responsible for any lost, stolen, or damaged personal property brought to or used during participation.

I grant the Released Parties the irrevocable right and permission to photograph, video record, and use my image, likeness, name, and voice in any media, including social media, marketing materials, sponsorship materials, advertising campaigns, and future event promotions without compensation or further approval.

I understand that no guarantees have been made regarding fitness results, safety outcomes, or overall experience.

I acknowledge that I have carefully read and fully understand this waiver and voluntarily agree to its terms. I understand that by signing this waiver, I am waiving substantial legal rights, including the right to sue.
