By completing my registration and attending the “Pilates & Lattes: Christmas Edition” event on December 20, 2025, I hereby acknowledge and agree to the following terms:

1. ASSUMPTION OF RISK

I understand that participation in physical fitness activities, including but not limited to Pilates Sculpt, stretching, meditation, sound healing, and any movement-based exercises, carries inherent risks.
These risks may include, but are not limited to:
• Muscle strains or sprains
• Joint injuries
• Cardiovascular events
• Dizziness or fainting
• Slips, trips, or falls
• Aggravation of existing medical conditions

I voluntarily choose to participate and assume full responsibility for any and all risks, whether known or unknown.

2. RELEASE OF LIABILITY

In consideration of being allowed to participate in this event, I release, waive, and discharge:
• IZXOTI Wellness
• Instructor Fanny Sanchez (also known as Estefania Sanchez)
• Scorpion Dance Studio (venue)
• All owners, staff, volunteers, employees, contractors, and affiliates

from any and all liability, claims, demands, actions, or causes of action related to:
• Injury
• Illness
• Death
• Property loss or damage
• Accidents occurring before, during, or after the event

whether caused by negligence or otherwise.

3. MEDICAL ACKNOWLEDGMENT

I confirm the following:
• I am physically able to participate in this event.
• I have no medical conditions that would make participation unsafe.
• I will inform the instructor of any limitations, injuries, or concerns before class.
• I understand that this event is not a substitute for medical advice and that I should consult with my physician before participating.

If medical attention is required, I authorize event staff or emergency personnel to provide assistance and assume full responsibility for any associated costs.

I understand that photos and videos may be taken during the event for marketing, social media, and promotional purposes.

• I accept all risks related to potential exposure during group activities.

I understand and agree to:
• Wear grip socks for my safety
• Follow the instructor’s guidance
• Use provided mats or my own at my discretion
• Respect studio rules and community guidelines

Failure to follow instructions may result in removal without refund.

8. INDEMNIFICATION

I agree to indemnify and hold harmless IZXOTI Wellness, Fanny Sanchez (Estefania Sanchez), and Scorpion Dance Studio against any claims brought by third parties arising from my participation in the event.

9. NO REFUNDS POLICY

All sales are final. No refunds will be issued for cancellations, lateness, or inability to attend.
If the organizer cancels the event, participants may receive a refund or credit toward a future event.
