Assumption of Risk
I understand that Pilates and related fitness activities involve physical movement and may include strength, flexibility, balance, breathing, and cardiovascular exercise. I acknowledge that participation carries risks of injury (including but not limited to muscle soreness, strains, sprains, or in rare cases, more serious injury). I voluntarily choose to participate in classes offered by FaithFlow Pilates and assume all risks associated with these activities.

Health Acknowledgment
I affirm that I am in good physical condition to participate in exercise activities. I have disclosed any relevant health conditions (such as heart issues, pregnancy, injuries, or other concerns) to my instructor. I agree to consult with my physician before beginning any new exercise program and take full responsibility for monitoring my health during participation.

Release of Liability
In consideration of being allowed to participate in Pilates classes, I hereby release and hold harmless FaithFlow Pilates, its instructors, affiliates, employees, and volunteers from any and all liability, claims, demands, or causes of action arising from my participation. I understand this waiver applies to both in-person and virtual classes.
