Participant Waiver of Liability and Assumption of Risk

I, the undersigned participant, understand and acknowledge that participation in Pilates and fitness-related activities involves inherent risks, including but not limited to, the risk of injury, illness, or other physical harm. By signing this waiver, I agree to the following:
	1.	Assumption of Risk
I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, that I may incur as a result of participating in this event.
	2.	Release of Liability
I release and hold harmless Pilatez, Fatemah Zaidi, event organizers, instructors, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to my participation in the event.
	3.	Medical Clearance
I certify that I am physically fit to participate and have consulted with a physician if necessary. I will notify the instructor of any pre-existing conditions or limitations before participating.

Media Release & Consent

I grant permission to Pilatez and its affiliates to photograph, video record, and/or otherwise capture my likeness during the event. I consent to the use of these images and recordings for promotional, marketing, and social media purposes without compensation.

I waive any right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears.
