BALANCEDAF EVENT WAIVER & RELEASE OF LIABILITY

By registering for and attending **sculptAF.** hosted by **BalancedAF by Izzy** at **Health House**, I acknowledge that I have read, understood, and voluntarily agree to the following:

Assumption of Risk

I understand that participation in Pilates, stretching, meditation, sound bath experiences, and wellness recovery modalities involves inherent risks, including but not limited to muscle strains, falls, slips, dizziness, dehydration, changes in blood pressure, fainting, allergic reactions, heat-related illness, cold-related injury, burns, aggravation of pre-existing conditions, and other risks associated with physical activity and wellness treatments.

I voluntarily assume all risks associated with participating in this event.

Physical Condition & Medical Responsibility

I certify that I am physically able to participate in Pilates, sound bath, and any optional wellness modalities offered during the event.

I understand that it is my responsibility to consult with my physician before participating if I have any medical concerns or conditions that may affect my ability to safely participate.

I agree to immediately discontinue participation in any activity if I experience pain, dizziness, discomfort, or any symptoms that may place my health at risk.

Wellness Modalities

I understand that this event includes optional access to Health House's wellness amenities, which may include, but are not limited to:

* Infrared Saunas
* Cold Plunges
* Vitamin C Showers
* Red Light Therapy
* Himalayan Salt Therapy
* Sound Bed
* Massage Chairs
* Vibration Therapy
* Compression Therapy
* Tanning Bed
* Recovery Equipment
* Any additional wellness services available at Health House during the event

I understand that participation in these modalities is **completely voluntary**.

I acknowledge that certain modalities may not be appropriate for individuals who are pregnant or who have cardiovascular disease, uncontrolled high or low blood pressure, respiratory conditions, seizure disorders, diabetes, implanted medical devices, recent surgeries, skin conditions, open wounds, or other medical conditions.

I accept full responsibility for determining whether each modality is appropriate for me and release BalancedAF and Health House from any liability arising from my voluntary participation.

Food, Beverage & Wellness Products

I understand that complimentary beverages and wellness products may be provided during the event, including but not limited to coffee, matcha, mocktails, wellness shots, hydration products, supplements, snacks, skincare products, samples, or promotional items provided by BalancedAF, Health House, or participating vendors.

I voluntarily consume or use these items at my own risk and acknowledge that I am solely responsible for identifying any allergies, dietary restrictions, food sensitivities, or medical conditions that may affect me.

Release of Liability

I release, waive, and forever discharge **BalancedAF by Izzy, Isabelle Claude, Health House, all instructors, sound bath facilitators, employees, contractors, volunteers, vendors, sponsors, affiliates, property owners, and event partners** from any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses arising out of or related to my participation in this event, including use of any wellness modality, except where prohibited by law.

Personal Property

I understand that BalancedAF and Health House are not responsible for lost, stolen, or damaged personal belongings or any damage to my vehicle or personal property while attending the event.

Photography & Video Release

I understand that photographs and videos may be taken throughout the event.

By attending, I grant BalancedAF, Health House, and participating partners permission to use my likeness in photographs and video recordings for marketing, promotional, advertising, website, email, and social media purposes without compensation.

Marketing Communications

By registering, I consent to receive future marketing emails and promotional communications from **BalancedAF** and participating event partners. I understand that I may unsubscribe at any time.

Age Requirement

I certify that I am **18 years of age or older**.

Acknowledgment

By registering for **sculptAF.**, I acknowledge that I have carefully read this Waiver and Release of Liability, fully understand its contents, and voluntarily agree to its terms. I understand that I am giving up certain legal rights by accepting this waiver and that it shall be binding upon me, my heirs, personal representatives, and assigns.
