By participating in Shake & Sip: Pom Cardio Dance + Social, I acknowledge and agree to the following:

### Assumption of Risk & Waiver

I understand that participation in dance, cardio, fitness, movement, and related activities involves inherent risks, including but not limited to muscle soreness, strains, sprains, falls, collisions, or other injury.

I confirm that I am physically able to participate and understand that I am responsible for determining whether participation is appropriate for me. I agree to listen to my body, modify movements as needed, and stop participating if I experience pain, dizziness, discomfort, or any other concerning symptoms.

In consideration of being permitted to participate, I voluntarily assume all risks associated with the event and release and hold harmless **Sparc Social Collective LLC, Sparc Collective, Sparc Social Squad, Sparc Dance Company, Into the Well Collective, event instructors, staff, volunteers, partners, and representatives** from claims, liabilities, damages, or expenses arising from my participation, except where prohibited by law.

I understand that this waiver applies to my participation in the event and any related activities taking place at the event venue.

### Media Release

I understand that photography, videography, and other forms of media may be captured during the event.

By attending, I grant **Sparc Social Collective LLC and Sparc Collective** permission to photograph, record, and otherwise capture my image, likeness, voice, and participation during the event.

I authorize Sparc to use this content for promotional, marketing, social media, website, advertising, and other business purposes without additional compensation or approval.

I understand that content may be edited, reproduced, published, distributed, and shared across Sparc's digital and social media platforms.
