SHINE ON WHEELS PARTICIPATION WAIVER & RELEASE OF LIABILITY

By participating in Shine on Wheels, I acknowledge and agree that I am physically able and medically fit to participate in the skating and other physical activities included in the event.

I understand that roller skating and participation in outdoor physical activities involve inherent risks, including but not limited to falls, collisions with other participants or objects, loss of balance, injuries, dizziness, illness, scrapes, bruises, fractures, or other physical harm. I voluntarily choose to participate and assume all risks associated with my participation.

I understand that Shine on Wheels takes place in an outdoor public space and that conditions may include uneven surfaces, pedestrians, cyclists, changing weather conditions, and other circumstances that may present additional risks. I agree to remain aware of my surroundings and to participate responsibly.

I understand that I am responsible for providing and using my own skates and any personal protective equipment I choose or need to use. I acknowledge that the event organizers are not responsible for the condition, fit, safety, or proper use of my personal equipment.

I agree to release and hold harmless the event organizers, Sip & Shine LLC, Herbalife, and their respective owners, employees, instructors, coaches, representatives, affiliates, and volunteers from any and all liability, claims, damages, losses, or injuries that may arise from or be related to my participation in Shine on Wheels, except where prohibited by law.

I understand that I should consult with a physician before participating if I have any medical condition, health concern, injury, or doubt about my ability to safely participate in roller skating or physical activity.

Please check with your physician before participating if any of the following apply to you:

• You have been prescribed medication for blood pressure or a heart condition.
• A physician has advised you to participate only in specific types of physical activity.
• You experience chest pain during physical activity.
• You have experienced chest pain within the past month while not participating in physical activity.
• You have lost your balance due to dizziness or have experienced unexplained fainting.
• You have a bone, joint, muscle, or other physical condition that could worsen with skating or physical activity.
• You are pregnant or have given birth within the past six months.
• There is any other reason why you believe you should not participate in skating or physical activity.

I understand that if I experience dizziness, weakness, lightheadedness, nausea, chest pain, shortness of breath, pain, or any other discomfort during the event, I must stop participating immediately and seek appropriate medical attention.

I understand that participation in Shine on Wheels is voluntary and that I am participating entirely at my own risk and responsibility.

PHOTO & VIDEO RELEASE

I grant the event organizers permission to photograph and/or record me duringShine on Wheels. I understand that these photographs and videos may be used for promotional, marketing, social media, advertising, and other business purposes without additional compensation or prior approval.

By completing and submitting this waiver, I confirm that I have read, understood, and voluntarily agreed to all of the statements and terms contained in this document. I acknowledge that I have had the opportunity to ask questions and that I am choosing to participate in Shine on Wheels voluntarily and at my own risk.
