Yoga

I, the undersigned, acknowledge that I am voluntarily participating in a yoga class, either in-person or online, and I understand that participation in yoga involves physical activity and may involve the risk of injury. I am aware that it is my responsibility to consult with my physician prior to participating in yoga or any other physical activity to ensure I am in good health and able to participate.

I grant permission for the instructor to take photographs, videos, or other media of me during the class for marketing, promotional, and educational purposes, unless I notify them in writing that I do not consent to such use.

By completing this form, the participant acknowledges that they have read, understood, and agree to the terms of this Waiver of Liability Agreement.
