PARTICIPANT LIABILITY WAIVER, ASSUMPTION OF RISK & RELEASE OF CLAIMS

By registering for and participating in this event, I acknowledge, understand, and voluntarily agree to the following terms:

1. Voluntary Participation & Assumption of Risk

I understand that participation in this event is voluntary and may include Mat Pilates, Soca dance, stretching, physical exercise, red light therapy, compression therapy, and other wellness activities.

I acknowledge that these activities involve inherent risks, including but not limited to muscle strains, falls, dizziness, dehydration, cardiovascular complications, aggravation of existing conditions, and other injuries or adverse reactions.

I voluntarily assume all known and reasonably foreseeable risks associated with my participation, including the risk of serious injury.

2. Health & Personal Responsibility

I confirm that I am voluntarily participating and am responsible for determining whether I am physically capable of engaging in these activities.

I agree to inform the instructor or appropriate service provider of any physical limitations or relevant health concerns, modify exercises as needed, follow safety instructions, and discontinue participation if I experience pain, dizziness, or discomfort.

I understand that event activities are not a substitute for professional medical advice.

3. Wellness Treatments & Medical Services

I understand that red light therapy and compression therapy may involve additional risks and may not be appropriate for everyone.

I acknowledge that any optional Vitamin B12 injection is a medical service subject to clinical screening, eligibility, and separate informed consent administered by an appropriately licensed healthcare professional.

This event waiver does not replace any medical consent, screening, or disclosure required by the healthcare provider.

4. Release of Liability & Waiver of Claims

TO THE FULLEST EXTENT PERMITTED BY FLORIDA LAW, I HEREBY RELEASE AND DISCHARGE RICH IN EXPERIENCE LLC AND THE LEGAL ENTITY OPERATING FORMÉ HEAT STUDIOS, including their respective owners, officers, employees, instructors, contractors, agents, and representatives (collectively, the “Released Parties”), from claims, demands, causes of action, damages, losses, or liabilities arising from my participation in the event, including claims arising from the ordinary negligence of the Released Parties.

This release applies to personal injury, illness, property damage, and other losses arising from event-related activities, facilities, or equipment.

This waiver does not release liability that cannot lawfully be waived under Florida law, including gross negligence, reckless conduct, or intentional misconduct.

5. Emergency Medical Assistance

In the event of an emergency, I authorize event personnel to contact emergency medical services. I understand that I am responsible for any resulting medical expenses to the extent permitted by law.

6. Acknowledgment & Agreement

I confirm that I am at least 18 years of age, have read and understood this agreement, and voluntarily accept its terms.

I understand that by accepting this waiver, I am giving up certain legal rights, including the right to bring claims against the Released Parties for their ordinary negligence, to the extent permitted by Florida law.

BY COMPLETING MY EVENT REGISTRATION AND AFFIRMATIVELY ACCEPTING THIS WAIVER, I ACKNOWLEDGE MY AGREEMENT TO ITS TERMS.
