In consideration of my participation, I release and hold harmless Soul Space Holistic Center, its owners, facilitators, staff, and partners from any and all liability, claims, demands, or causes of action arising from my participation, including but not limited to personal injury, property damage, allergic reaction, or any other consequence.

Consent

I consent to receive holistic services including but not limited to yoga instruction, guided meditation, sound healing, EFT tapping, acupuncture, and herbal beverages. I understand my participation is voluntary, and I may decline or stop any activity at any time.

Photography/Media Release

I grant permission to be photographed or recorded during the event, and for such media to be used for promotional or educational purposes by Soul Space Holistic Center. ( ) I opt out.
