### PARTICIPANT WAIVER, RELEASE OF LIABILITY & MEDIA CONSENT

By signing this waiver, I acknowledge and agree to the following:

### 1. ASSUMPTION OF RISK

I understand that participation in dance classes, rehearsals, movement activities, stretching, conditioning, and related physical activities involves inherent risks, including but not limited to muscle strains, sprains, falls, collisions, physical exertion, or other injury.

I voluntarily choose to participate in the Sparc Dance Co. Season 2 Prep Class and understand that I am responsible for determining whether I am physically capable of participating safely.

I agree to listen to and follow the instructions of Sparc Dance Co. Director, coaches, instructors, staff, and event personnel and to stop participating if I experience pain, discomfort, illness, or injury.

### 2. RELEASE OF LIABILITY

To the fullest extent permitted by applicable law, I voluntarily release and agree to hold harmless **Sparc Social Collective LLC, Sparc Collective, Sparc Dance Co., its owners, directors, coaches, instructors, staff, volunteers, contractors, representatives, and affiliated partners** from claims, demands, damages, losses, liabilities, costs, or expenses arising out of or related to my participation in the event, including claims relating to personal injury, property damage, or loss, except to the extent caused by conduct that cannot legally be released.

I understand that this release applies to activities occurring before, during, and after the scheduled class in connection with my participation in the event.

### 3. MEDICAL RESPONSIBILITY

I represent that I am voluntarily participating and that I am responsible for determining whether participation is appropriate for me based on my own physical condition and abilities.

I agree to immediately notify an instructor or event representative if I become injured, feel unwell, or require assistance.

In the event of an emergency, I authorize Sparc Dance Co. and its representatives to seek reasonable emergency medical assistance on my behalf when necessary. I understand that I am responsible for any medical expenses incurred on my behalf.

### 4. MEDIA RELEASE & CONSENT

I understand that photography, video, audio recording, and other forms of media may be captured during the Sparc Dance Co. Season 2 Prep Class.

By signing below, I grant **Sparc Social Collective LLC, Sparc Collective, and Sparc Dance Co.** permission to photograph, film, record, and otherwise capture my image, likeness, voice, and participation during the event.

I authorize Sparc to use this content for legitimate business, promotional, marketing, advertising, social media, website, portfolio, and other communications purposes, without additional compensation or approval.

This permission includes, but is not limited to, use on:

* Instagram
* TikTok
* Facebook
* YouTube
* Sparc websites and digital platforms
* Promotional materials
* Event recaps
* Advertisements
* Future Sparc Dance Co. and Sparc Collective marketing

I understand that I will not receive compensation for the use of my image, likeness, or recordings.

### 5. OWNERSHIP OF MEDIA

I understand that all photographs, videos, recordings, and other media captured by or on behalf of Sparc during the event may be owned by Sparc Social Collective LLC or its designated photographers, videographers, or media partners.

I waive any right to inspect or approve the final photographs, videos, edits, captions, or other materials in which I may appear, to the extent permitted by law.

### 6. PERSONAL BELONGINGS

I understand that I am responsible for my own personal belongings brought to the event. Sparc Dance Co. and its representatives are not responsible for lost, stolen, or damaged personal property, except to the extent such responsibility cannot legally be excluded.

### 7. ACKNOWLEDGMENT

I have read this waiver and understand that I am giving up certain legal rights by signing it. I have had the opportunity to ask questions regarding this agreement and voluntarily agree to its terms.

I certify that the information provided above is accurate and that I am signing this agreement voluntarily.
