ROLLERINA SKATE CO. – LIABILITY WAIVER & RELEASE FORM

PLEASE READ CAREFULLY BEFORE SIGNING

This Liability Waiver & Release of Claims (“Agreement”) is entered into by the participant (or parent/guardian if under 18) in consideration of being permitted to participate in all Rollerina Skate Co. activities, including but not limited to skating sessions, routes, lessons, events, and meetups.

1. ASSUMPTION OF RISK

I understand that roller skating and related activities are physical in nature and carry inherent risks, including but not limited to:

* Falls, collisions, or contact with other participants
* Uneven surfaces, pavement, or environmental hazards
* Equipment failure or improper use
* Injury ranging from minor bruises to serious injury or death

I voluntarily choose to participate and accept all risks associated with these activities.

2. RELEASE OF LIABILITY

I hereby release, waive, discharge, and hold harmless Rollerina Skate Co., its founder(s), instructors, volunteers, staff, and affiliates from any and all claims, liabilities, or damages arising from participation in any Rollerina Skate Co. activity.

This includes claims caused by negligence to the fullest extent permitted by law.

3. MEDICAL ACKNOWLEDGMENT

I confirm that I am physically able to participate in skating activities. I understand it is my responsibility to consult a medical professional if I have any concerns.

I authorize emergency medical treatment if needed and understand I am responsible for any related costs.

4. MINORS (IF APPLICABLE)

If signing for a participant under 18:

* I certify I am the parent/legal guardian
* I consent to their participation
* I accept full responsibility for their actions and safety during participation

5. CODE OF CONDUCT

I agree to follow all safety instructions, respect instructors and participants, and skate responsibly. Rollerina Skate Co. reserves the right to remove any participant who violates safety rules.

6. MEDIA RELEASE 

I give permission for photos/videos of me (or my child) to be used for Rollerina Skate Co. social media, marketing, and promotional content.

By signing below, I confirm:

* I am signing voluntarily
* I agree to release Rollerina Skate Co. from liability as stated above
