Hello! We'd like to invite you to join our VIRAL speed dating pickleball tournament!

You'll move through matches, rotating partners after each round. Two winners will be crowned most eligible Bachelor and Bachelorette. After the Tournament we'll head across the street to a local bar and continue our night!

When: April 12th 3:45pm - 6pm;  Evening continues at Jonahs Kitchen and Bar after!&#x20;
Where: Santa Monica Pickleball Center
**Age: 25 - 35 (this is the recommend age range)**

Various drinks available for your enjoyment!&#x20;

You will be emailed more details around parking and tournament format as the date approaches.&#x20;

Pickleball

LIABILITY:

For and in consideration of Pickle N Tequila, permitting me to enroll in and participate in pickleball classes or related activities (collectively, the “Activity”), by signing below I hereby voluntarily indemnify, release from liability and hold harmless Pickle N Tequila, and its owner(s), coaches, agent(s) and contractor(s) (collectively, “Pickle N Tequila”) for any accident, injury, illness, death, loss, damage to person or property, or other consequences suffered by me or any other person arising or resulting directly or indirectly from my participation in the Activity.

If I am injured, I agree to assume any financial obligation, either through my personal health insurance, or through some other means, for any medical costs which I incur. Pickle N Tequila assumes no responsibility for any medical expenses, injury, or damage suffered by a Participant in connection with the use of any Pickle N Tequila facilities/locations or services in connection with the Activity (regardless of where such services take place).

I further agree to conform to all rules and regulations adopted by Pickle N Tequila and/or any other facility/location at which the Activity takes place (e.g., tennis courts, pickleball courts, facilities, public parks, public facilities, private locations, etc.)

It is my intention by signing below to expressly assume all risk of personal injury, death, or property damage upon myself, to the exclusion of Pickle N Tequila and any of it’s coaches, contractors, and to exempt and relieve Pickle N Tequila from liability for personal injury, property damage or wrongful death. If events are provided at a private residence, I agree to hold the owner of such residence as well as Pickle N Tequila, and it’s contractors and owner(s), additionally harmless from liability, loss, or damage arising at such residence.

I agree that neither I nor my spouse, assignees, heirs, guardians, and/or legal representatives will make any claim against, sue or attach the property of Pickle N Tequila and/or any owner or coach or agent thereof, for any loss or damage resulting from my participation in the Activity.

I am aware of the potential dangers incidental to the activity, that this is a release of liability, a waiver of my legal right to collect damages in the event of injury, death or property damage and a contract between myself and Pickle N Tequila, and I voluntarily sign this document.

PERSONAL RESPONSIBILITY WITH ALCOHOL:

Consumption of alcohol at events is optional and a personal decision. By attending an event by Pickle N Tequila, I agree that any alcoholic beverages I consume are my own choice and responsibility. I agree to drink responsibility and heed suggestions from others and will not drive if I am intoxicated.

Pickle N Tequila, its owners, operators, employees, agents, and servants are not responsible or liable for any damages to myself or third parties that result from the consumption of alcohol by myself or others, to include, but not limited to, property damage, personal injury, death, disfigurement, or loss of consortium.

That should I consume alcohol or food products at an event hosted by Pickle N Tequila, I acknowledge that I am solely responsible for any and all damages suffered by myself or that I may cause to third parties as a result of said consumption to include, but not limited to, property damage, personal injury, death, disfigurement, or loss of consortium.

I acknowledge it is against the law to consume, serve, provide, or make available alcohol to anyone under the age of twenty-one (21). I swear and affirm not to provide anyone under the age of twenty-one (21) with alcohol.

CANCELLATION AND NO SHOW POLICY:

We understand that situations arise in which you must cancel your appointment. All sales are final, however if you can no longer make it we will do our best to find a replacement and refund you. If your cancelation is within 24 hours we cannot offer a refund. Thank you for understanding.


MEDIA CONTENT:

I understand that photos or videos may be taken during the course of my involvement during Pickle N Tequila, which may be used for promotional purposes.


RULES AND/OR REGULATIONS:

Members who do not observe Pickle N Tequila rules and regulations or who abuse equipment in any fashion will be asked to leave. The management reserves the right to terminate membership of anyone who refuses to observe any of Pickle N Tequila rules or regulations. Not all rules and regulations are listed in this agreement. Pickle N Tequila reserves the right to add, change or remove rules, conditions of membership, opening and closing hours, and all services and facilities offered by Pickle N Tequila.

Member has read, and fully agrees to the term of this Agreement and understands and agrees that by signing this Agreement (which contains a waiver, release and assumption of risks) Member has given up considerable future legal rights. Member has signed this Agreement freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated to him/her. Member certifies and warrants that he/she is 18 years of age or older and mentally competent to enter into this Agreement.

PRIVATE LESSON LIABILITY DISCLAIMER AND BOOKING POLICY

Pickle N Tequila wishes to inform all members that we are not liable for any private lessons arranged or conducted outside of official Pickle N Tequila bookings. We emphasize that all events or lessons conducted with Pickle N Teuilqa must be directly booked through our authorized channels and payment links. Pickle N Tequila does not assume any responsibility for lessons taken outside of our official business.

IDEMNIFICATION AND HOLD HARMLESS:

I also agree to INDEMNIFY AND HOLD HARMLESS Pickle N Tequila, the Owner and Organizers, their agents and representatives from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees brought as a result of my participation in Pickle N Tequila.

 

I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.
