A few important notes:
• Each person must purchase their own ticket and sign their own waiver
• You cannot sign for someone else
• Please bring your own mat, water bottle, and scrunchie if needed
• Weights are optional, if you’d like to bring some, I recommend 2–5 lb dumbbells (You can find affordable ones at Marshalls, TJ Maxx, or Five Below)

Also, just to clarify! these are weekend classes, not special events. Events will be separate and announced differently.

Activities Covered: sculpt Pilates, pilates, barre, Yoga, Sculpt, Strength Training, Stretching, Group Fitness, Workshops, Wellness Events, Outdoor Events, Private Sessions, and all related physical activities.

1. ACKNOWLEDGMENT OF VOLUNTARY PARTICIPATION

I acknowledge that I am voluntarily participating in activities hosted by IZXOTI Wellness (“Company”). Activities may take place in rented studios, commercial facilities, private residences, outdoor parks, event venues, or other locations not owned, operated, or controlled by Company.

Company does not own, maintain, inspect, control, or guarantee the safety of any third-party location.

2. ASSUMPTION OF ALL RISKS

I understand that fitness and wellness activities involve inherent and unforeseeable risks, including but not limited to:
	•	Muscle strains, tears, sprains
	•	Broken bones
	•	Falls
	•	Equipment malfunction
	•	Slipping hazards
	•	Overexertion
	•	Dizziness or fainting
	•	Cardiac events
	•	Dehydration
	•	Heat-related illness
	•	Aggravation of pre-existing conditions
	•	Allergic reactions
	•	Food-borne illness

I knowingly and voluntarily assume ALL risks, both known and unknown, foreseeable and unforeseeable, even if arising from the ordinary negligence of Company or any released party.

3. MEDICAL REPRESENTATION

I certify that:
	•	I am physically capable of participation.
	•	I have no condition that would prevent safe participation.
	•	I have consulted a medical professional if necessary.
	•	I will immediately stop activity if I experience pain, dizziness, or distress.

Company is not a healthcare provider.

4. RELEASE OF LIABILITY (INCLUDING NEGLIGENCE)

TO THE FULLEST EXTENT PERMITTED BY LAW, I HEREBY RELEASE, WAIVE, DISCHARGE, AND AGREE NOT TO SUE:

	•	Estefania Sanchez
	•	Any assistants, contractors, or volunteers
	•	Sponsors
	•	Product manufacturers
	•	Event collaborators
	•	Venue owners
	•	Landlords
	•	Property managers
	•	Studio operators
	•	Hosts of private residences
	•	Any affiliated third parties

(collectively referred to as “Released Parties”)

FROM ANY AND ALL CLAIMS, DEMANDS, DAMAGES, CAUSES OF ACTION, OR LIABILITY ARISING OUT OF OR RELATED TO PARTICIPATION, INCLUDING CLAIMS ARISING FROM THE ORDINARY NEGLIGENCE OF ANY RELEASED PARTY.

This release does not apply to gross negligence or willful misconduct where prohibited by law.

5. THIRD-PARTY VENUE & INDEPENDENT STATUS

I understand and agree that IZXOTI Wellness operates independently and is not affiliated with, employed by, or legally responsible for any venue, studio, park district, private home, landlord, or facility where events are conducted.

Participation does not create any partnership, joint venture, or agency relationship.

6. FOOD, BEVERAGE & PRODUCT LIABILITY WAIVER

Food, beverages, supplements, energy drinks, wellness products, or sponsored items may be provided.

I acknowledge and agree:
	•	I am solely responsible for reviewing ingredients.
	•	I am responsible for managing allergies or sensitivities.
	•	I consume any food or beverage at my own risk.
	•	Company makes no warranties regarding safety, preparation, storage, or ingredient disclosure.
	•	Company is not responsible for adverse reactions, illness, contamination, or manufacturer defects.

Any claims related to products must be directed to the manufacturer.

7. INDEMNIFICATION

I agree to indemnify, defend, and hold harmless the Released Parties from and against any claims, lawsuits, damages, attorney’s fees, or costs arising out of:

	•	Damage I cause to property

8. PROPERTY DAMAGE RESPONSIBILITY

I agree to be financially responsible for any damage I cause to equipment, props, studio property, private residences, outdoor locations, or event spaces.

9. NO REFUND POLICY

ALL SALES ARE FINAL.

I acknowledge and agree that there are NO REFUNDS for any reason, including but not limited to:
	•	Illness
	•	Injury
	•	Weather
	•	Personal emergency
	•	Scheduling conflict
	•	Dissatisfaction
	•	Travel issues
	•	Failure to attend

By registering, I accept this no-refund policy.

10. PHOTOGRAPHY & MEDIA RELEASE

I grant permission for Company to use photographs or video recordings taken during events for promotional, marketing, and social media purposes without compensation.

If I do not consent, I must notify Company in writing prior to participation.

11. SEVERABILITY

If any provision of this agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.

This Agreement shall be governed by the laws of the State of Illinois.
