LIABILITY WAIVER AND RELEASE OF CLAIMS (FLORIDA)

By signing below and/or registering for this event, I acknowledge that I am voluntarily participating in Pilates classes, wellness activities, Bible study discussions, community gatherings, and related events offered by PraiseForm Pilates and its instructor(s), including but not limited to activities held at beaches, parks, studios, or other indoor or outdoor locations.

I understand that participation in physical exercise and outdoor activities involves inherent risks, including but not limited to muscle strain, falls, dehydration, weather-related conditions, ocean or beach hazards, illness, allergic reactions, physical injury, property damage, or other health complications. I voluntarily and knowingly assume all risks, known or unknown, associated with my participation.

I affirm that I am physically capable of participating and have no medical condition that would prevent safe participation. I understand that it is my responsibility to consult with a physician prior to participating if I have any health concerns or medical conditions.

In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless PraiseForm Pilates, its owner (Liz Nguyen), instructors, contractors, volunteers, sponsors, venue partners, affiliates, and any associated entities from any and all liability, claims, demands, actions, damages, losses, costs, or causes of action arising out of or related to any injury, illness, loss, damage, accident, delay, or death that may occur during or as a result of participation in these activities, whether caused by negligence or otherwise, to the fullest extent permitted under Florida law.

I acknowledge that some events may take place outdoors, including at beaches and parks, and I voluntarily assume all risks associated with outdoor environments, including but not limited to uneven terrain, sand, wildlife, ocean conditions, sun exposure, weather conditions, traffic, and other natural or environmental hazards.

I consent to receive first aid or emergency medical treatment if deemed necessary during participation, and I understand that I am solely responsible for any medical expenses incurred.

I understand that this event may include a voluntary faith-based or Bible study component. Participation in discussions is voluntary, and all attendees are expected to maintain a respectful and inclusive environment.

EVENT POLICIES

Refunds & Cancellations:
Tickets, registrations, and donations are generally non-refundable unless the event is canceled by the organizer. PraiseForm Pilates reserves the right to reschedule, modify, relocate, or cancel any event due to weather, safety concerns, low attendance, venue issues, or unforeseen circumstances.

If refunds are issued, attendees acknowledge that payment processing fees charged through Stripe or third-party platforms (typically including processing fees such as 2.9% + $0.30) may be non-refundable and may be deducted from the refunded amount in accordance with platform and payment processor policies.

In the event of cancellation, refunds, credits, or rescheduling options may be offered at the organizer’s discretion and in accordance with applicable platform policies.

Ticket Transfers:
Tickets may be transferred to another participant with prior organizer approval.

Code of Conduct:
Participants agree to behave respectfully toward organizers, instructors, volunteers, attendees, and the surrounding community. Disruptive, unsafe, discriminatory, intoxicated, or inappropriate behavior may result in removal from the event without refund.

Personal Property:
Participants are solely responsible for their personal belongings. PraiseForm Pilates is not responsible for lost, stolen, or damaged items.

Media Release:
I grant permission to PraiseForm Pilates to use photographs, videos, or recordings taken during classes or events for promotional, marketing, website, and social media purposes unless I notify the organizer in writing prior to participation.

I acknowledge that I have carefully read and fully understand this waiver and release of liability. I understand that by signing this document, I am waiving certain legal rights, including the right to sue. I sign this agreement freely and voluntarily.
