Pilates

This Pilates Waiver Agreement ("Agreement") is made between the participant ("Participant") and Dohpuhmeen Wellness, Mikayla McCall ("Company"), concerning the Participant's engagement in Pilates and Wellness events, classes, sessions, or related activities offered by the Company.

1. Acknowledgment of Risk
I, the undersigned, understand and acknowledge that participation in Pilates and Wellness events involves inherent risks, including but not limited to physical exertion, strains, sprains, muscle injuries, and other risks associated with physical activity. I recognize that while Pilates is generally low-impact, it may still involve risks of injury.

2. Health and Physical Condition
By signing this Agreement, I confirm that:
I am in good health and have no physical or mental conditions that would prevent safe participation in Pilates and Wellness Events.

3. Voluntary Participation
I agree to participate in Pilates classes, sessions, or wellness events voluntarily and assume full responsibility for any injury, loss, or damage resulting from my participation. I understand that it is my responsibility to perform exercises only to the extent that I am able to do so without discomfort or pain.

4. Liability Waiver and Release
To the fullest extent permitted by law, I hereby release and hold harmless Dohpuhmeen Wellness Events, Mikayla McCall, its instructors, employees, and agents, from any and all liability, claims, demands, or causes of action that I or my representatives may have arising from injury, illness, or damages related to my participation in Pilates activities or Wellness Events.

5. Photo and Video Release 
I give my permission to Dohpuhmeen Wellness, Mikayla McCall to use photographs or video images taken of me during classes for promotional, advertising, or marketing purposes.
 
6. Entire Agreement
This Agreement constitutes the entire agreement between Dohpuhmeen Wellness, Mikayla McCall, and the Participant and supersedes any previous discussions, representations, or agreements.

Acceptance and Signature
By obtaining a ticket, I affirm that I have read, understood, and agreed to the terms of this Agreement. I acknowledge that I am signing voluntarily and am of legal age.
