PLEASE READ THIS DOCUMENT CAREFULLY BEFORE SIGNING. THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS.

1. ACKNOWLEDGMENT OF PARTICIPATION

I acknowledge that I am voluntarily participating in The Afterglow: Dry January Happy Hour (the "Event") hosted by Renew Collective at Groundfloor Club Oakland (the "Venue") on January 24, 2026. I understand that the Event involves sampling functional beverages, food, and participating in social activities.

2. ASSUMPTION OF RISK

I understand and acknowledge that participation in this Event carries inherent risks, including but not limited to:

1. Consumption of functional beverages containing ingredients such as kava, adaptogens, botanicals, and other supplements that may cause allergic reactions or adverse health effects
2. Consumption of food that may contain allergens or cause adverse reactions
3. Potential interactions between functional beverage ingredients and medications, supplements, or pre-existing health conditions
4. Physical injury from slips, trips, falls, or other accidents at the Venue
5. Exposure to communicable diseases, including COVID-19 and other illnesses
6. Any other risks associated with attending a public gathering

I voluntarily assume all risks associated with my participation in the Event, whether known or unknown.

3. HEALTH AND MEDICAL DISCLAIMERS

Important Health Information:

1. The functional beverages served at this Event contain various botanical ingredients, adaptogens, and supplements that may not be suitable for everyone.
2. These beverages are NOT intended to diagnose, treat, cure, or prevent any disease and have not been evaluated by the FDA.
3. I acknowledge that I am responsible for reviewing all ingredient lists and making informed decisions about what I consume based on my own health status, allergies, and medical conditions.
4. I understand that kava and certain adaptogens may cause drowsiness, affect liver function, or interact with medications. I should consult my healthcare provider if I have concerns.
5. I certify that I am not pregnant, nursing, or have been advised by a medical professional to avoid consuming botanical supplements or adaptogens.
6. I will not consume any beverage or food if I am uncertain about the ingredients or if I have allergies or sensitivities.

4. ALLERGIES AND DIETARY RESTRICTIONS

I acknowledge that the food and beverages served may contain common allergens including but not limited to nuts, dairy, gluten, soy, and other ingredients. I am solely responsible for inquiring about ingredients and making safe choices for my dietary needs. I will not hold the Event organizers, Venue, or beverage brands liable for any allergic reactions or adverse effects.

5. RELEASE AND WAIVER OF LIABILITY

In consideration for being permitted to participate in the Event, I hereby RELEASE, WAIVE, DISCHARGE, and COVENANT NOT TO SUE Renew Collective, its founder Rachel, Groundfloor Club Oakland, the Venue owners and operators, and all participating beverage brands including Leilo, Curious Elixirs, alice, Corpse Reviver, and any other sponsors or vendors (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in the Event, whether caused by negligence of the Released Parties or otherwise.

6. INDEMNIFICATION

I agree to INDEMNIFY and HOLD HARMLESS the Released Parties from any loss, liability, damage, or costs, including attorney fees, that may incur due to my participation in the Event, whether caused by my own actions or the actions of the Released Parties.

7. VENUE RULES AND COMPLIANCE

I agree to comply with all rules, regulations, and policies of Groundfloor Club Oakland. I understand that I may be asked to leave the Venue if I violate these rules or behave in a manner that is disruptive, unsafe, or inappropriate. The Venue reserves the right to refuse service or entry to any individual.

8. PHOTO AND MEDIA RELEASE

I grant permission to Renew Collective, the Venue, and participating brands to use photographs, videos, or other media of me taken during the Event for promotional, marketing, or educational purposes without compensation. If I do not wish to be photographed or filmed, I will notify the Event organizers upon arrival.

9. COVID-19 AND HEALTH SAFETY

I acknowledge that attending the Event may expose me to communicable diseases, including COVID-19. I voluntarily assume all risks related to exposure and agree not to hold the Released Parties liable for any illness or infection contracted as a result of my attendance.

10. SLIDING SCALE PAYMENT

I understand that this Event operates on a sliding-scale payment model (suggested $10-20). I agree to pay what I am able and acknowledge that my payment is non-refundable.

11. SEVERABILITY

I understand that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.

12. ACKNOWLEDGMENT OF UNDERSTANDING

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
