PARTICIPANT WAIVER, ASSUMPTION OF RISK, AND RELEASE OF LIABILITY

I voluntarily choose to participate in the Flow by Kyrt yoga class held at lululemon Grand Boulevard on October 3, 2026 (“Event”).
I understand that yoga and other physical activities involve inherent risks, including strains, sprains, falls, dizziness, dehydration, aggravation of existing conditions, serious injury, and, in rare cases, death. These risks may result from physical exertion, equipment, the event space, outdoor or environmental conditions, other participants, or my own actions. I knowingly and voluntarily accept and assume all risks associated with participating in the Event, whether known or unknown.

I confirm that I am physically able to participate. If I am pregnant, injured, have a medical condition, or have concerns about my ability to exercise, I understand that it is my responsibility to consult a qualified healthcare professional before participating. I agree to listen to my body, modify or stop participating when needed, and notify the instructor of any relevant injury or limitation. I understand that the instructor does not provide medical advice.

TO THE FULLEST EXTENT PERMITTED BY FLORIDA LAW, I RELEASE AND HOLD HARMLESS KYRSTEN “KYRT” SCHNEIDER, FLOW BY KYRT, LULULEMON GRAND BOULEVARD, GRAND BOULEVARD, FORMULA FRESH, THE PROPERTY OWNER, AND THEIR RESPECTIVE OWNERS, EMPLOYEES, REPRESENTATIVES, AGENTS, VOLUNTEERS, AFFILIATES, SPONSORS, AND EVENT PARTNERS (“RELEASED PARTIES”) FROM CLAIMS FOR INJURY, ILLNESS, DEATH, LOSS, OR PROPERTY DAMAGE ARISING FROM OR RELATED TO MY PARTICIPATION IN THE EVENT, INCLUDING CLAIMS ARISING FROM THE ORDINARY NEGLIGENCE OF A RELEASED PARTY, EXCEPT TO THE EXTENT SUCH A RELEASE IS PROHIBITED BY LAW.

If I experience a medical emergency and cannot provide consent, I authorize the Event organizers to contact emergency medical services. I understand that I am responsible for any medical expenses incurred on my behalf.

I understand that consuming any food or beverage offered at the Event, including Formula Fresh juices, is optional. I am responsible for reviewing ingredients and determining whether a product is appropriate for me, including in relation to food allergies, dietary restrictions, or medical conditions.

I have carefully read and understand this agreement. I understand that I am giving up certain legal rights by accepting it. I accept this agreement knowingly and voluntarily and agree that my electronic acceptance has the same effect as a handwritten signature. I certify that I am at least 18 years old.
