By participating in this wellness session, class, workshop, or event hosted by Healing Mind, Body & Soul Wellness Group, Augire Balance, Gastby House of Wellness, Yoga MB and their collaborators (collectively, the "Organizers"), I acknowledge and agree to the following:
I understand that Pilates, yoga, breathwork, stretching, posture training, wellness activations, recovery sessions, wellness mocktails, refreshments, and related activities involve physical movement and participation and may carry risks including, but not limited to, injury, dizziness, discomfort, falls, allergic reactions, food or beverage sensitivities, muscle soreness, aggravation of existing conditions, or other unforeseen risks.
I certify that I am voluntarily participating and that I am physically able to participate in these activities. I understand it is my responsibility to consult with a physician before participating if I have any medical concerns, injuries, cardiovascular conditions, respiratory conditions, pelvic floor conditions, allergies, skin sensitivities, dietary restrictions, or any other health-related issues that may affect my participation.
I voluntarily assume all risks associated with participation and fully release, waive, discharge, and hold harmless Healing Mind, Body & Soul Wellness Group, Augire Balance, all instructors, assistants, volunteers, vendors, exhibitors, sponsors, collaborators, event organizers, promoters, contractors, affiliates, venue operators, venue owners, landlords, property owners, and property managers from any and all liability, claims, demands, causes of action, damages, injuries, losses, costs, or expenses arising out of or related to participation in this event, including claims arising from negligence, to the fullest extent permitted by the laws of the State of Florida.
I understand that this event is educational and wellness-based only and does not provide medical diagnosis, treatment, therapy, rehabilitation services, or medical advice.
I also grant permission to the Organizers and their collaborators, sponsors, vendors, and affiliates to photograph, record, and use my image, likeness, voice, and content captured during the event for marketing, promotional, website, social media, educational, and advertising purposes without compensation or further approval.
Participant Acknowledgment
By signing below, I acknowledge and agree that:
I am at least eighteen (18) years of age.
I have carefully read and fully understand this Waiver, Release of Liability, Assumption of Risk, and Media Consent Agreement.
I understand that by signing this agreement I am giving up certain legal rights, including the right to sue.
I voluntarily agree to all terms and conditions contained herein.
I understand that this agreement is intended to be as broad as permitted under the laws of the State of Florida.
