Liability Waiver and Photo/Video Release Agreement

By checking the box, I hereby acknowledge and agree as follows:

1. Assumption of Risk:
    I understand that participation in any program, treatment, or service provided by Body by MJ may involve inherent risks, including but not limited to physical injury or other harm. I voluntarily assume all risks associated with my participation.

2. Release of Liability:
    In consideration for being permitted to participate in services provided by Body by MJ, I hereby waive, release, and discharge Body by MJ, its owners, employees, agents, and representatives from any and all claims, liabilities, or demands arising from my participation. This release applies to all claims, whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Indemnification:
    I agree to indemnify and hold harmless Body by MJ from any and all claims, costs, damages, or expenses (including attorney’s fees) incurred as a result of my participation in the program, treatment, or service.

4. Photo/Video Release:
    I grant Body by MJ the unrestricted right to capture my image, likeness, voice, or any other representation via photographs and/or video recordings during the course of services. I consent to the use of such media for marketing, promotional, or educational purposes, including publication on websites, social media, and other promotional materials. I understand that I will not receive any compensation for the use of these images or videos, and I waive any right to inspect or approve the finished product.

5. Confidentiality and Privacy:
    Body by MJ agrees to maintain the confidentiality of all personal and medical information shared during the course of my participation, except as otherwise required by law. I acknowledge that the media captured may be shared publicly and that I have no expectation of privacy regarding the images or recordings.

6. Acknowledgment of Understanding:
    I certify that I have read and fully understand this Agreement. I acknowledge that I am signing this Agreement voluntarily and that no oral representations, statements, or inducements apart from the foregoing written agreement have been made.

7. Governing Law:
    This Agreement shall be governed by and construed in accordance with the laws of the state in which Body by MJ operates, without regard to any conflict of law provisions.
