By registering for and/or attending this event, I, the undersigned participant, acknowledge and agree to the following:

1. VOLUNTARY PARTICIPATION
I understand that my participation in this event, including but not limited to chair massage services, wine tasting, beauty activations, and all associated activities, is entirely voluntary.

2. ASSUMPTION OF RISK
I acknowledge that participation in this event involves certain risks, including but not limited to physical discomfort, allergic reactions to food, wine, or beauty products, and sensitivity to massage therapy. I voluntarily assume all such risks associated with my participation.

3. HEALTH & MEDICAL DISCLOSURE
I confirm that I am in good health and am not aware of any medical condition, injury, or pregnancy that would prevent my safe participation. I understand it is my responsibility to inform service providers of any allergies, sensitivities, or medical concerns prior to participating in any activity.

4. ALCOHOL ACKNOWLEDGMENT
I confirm that I am of legal drinking age (21+) in the state of Florida. I agree to consume alcohol responsibly and understand that Barrel Wine & Cheese Wine Bar and Restaurant and event organizers are not liable for any incidents resulting from alcohol consumption.

5. RELEASE OF LIABILITY
In consideration of being permitted to participate in this event, I hereby release, waive, and discharge Barrel Wine & Cheese Wine Bar and Restaurant, Haus of Wellness, their owners, organizers, employees, contractors, and agents from any and all claims, demands, or causes of action arising out of or related to my participation in this event, including any injury, loss, or damage to my person or property.

6. MEDIA RELEASE
I grant permission for photos and/or videos taken during the event to be used for promotional and social media purposes by the event organizers and Barrel Wine & Cheese Wine Bar and Restaurant.

7. INDEMNIFICATION
I agree to indemnify and hold harmless the event organizers and venue from any claims arising from my own negligence or misconduct during the event.
