5. MEDICAL RESPONSIBILITY I CERTIFY THAT I AM PHYSICALLY ABLE TO PARTICIPATE AND WILL SEEK MEDICAL CLEARANCE IF NECESSARY. I AGREE THAT I AM SOLELY RESPONSIBLE FOR ANY MEDICAL CONDITIONS, TREATMENTS, OR EMERGENCIES THAT MAY OCCUR.
6. MEDIA RELEASE I GRANT PERMISSION FOR THE BEAUTY BOOST FORT LAUDERDALE TO PHOTOGRAPH, FILM, OR OTHERWISE RECORD THE EVENT AND TO USE MY IMAGE, LIKENESS, AND VOICE FOR MARKETING, PROMOTIONAL, OR COMMERCIAL PURPOSES WITHOUT COMPENSATION.
7. NON-REFUNDABLE TICKET POLICY I UNDERSTAND THAT ALL TICKET SALES ARE FINAL AND NON-REFUNDABLE FOR ANY REASON, INCLUDING BUT NOT LIMITED TO ILLNESS, SCHEDULING CONFLICTS, PERSONAL EMERGENCIES, WEATHER, OR FAILURE TO ATTEND. NO EXCEPTIONS WILL BE MADE.
