1. Voluntary Participation

I acknowledge that my participation in The Wellness Kickback is voluntary. Activities may include, but are not limited to, stretching, Pilates, HIIT, walking, and other fitness or wellness-related exercises. I affirm that I am voluntarily choosing to participate and understand the nature of these activities.

2. Assumption of Risk

I understand that participation in physical and wellness-related activities involves inherent risks, including but not limited to: slips, falls, dehydration, allergic reactions, illness, injury, or other unforeseen events. I knowingly and freely assume all risks, both known and unknown, associated with participation.

3. Medical Clearance & Age Requirement

I affirm that I am at least 18 years of age, in good physical health, and have no medical condition that would prevent safe participation. I understand it is my responsibility to consult with a physician before participating in any physical activity if I have concerns regarding my health or ability.

4. Release of Liability

In consideration of being allowed to participate in The Wellness Kickback, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, and discharge The Wellness Kickback Inc., affiliated partners, vendors, volunteers, instructors, and sponsors from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, illness, or injury that may be sustained during the event.

5. Food & Beverage Acknowledgment

I understand that complimentary refreshments may be served and that it is my responsibility to be aware of my own food allergies, sensitivities, or dietary restrictions. I voluntarily choose to consume any food or beverages at my own risk.

6. Photography & Media Release

I grant permission for my image, likeness, and/or voice to be recorded, photographed, and used in promotional, educational, or marketing materials related to The Wellness Kickback, without compensation or further approval.

7. Emergency Medical Consent

In the event of a medical emergency, I consent to receive medical treatment deemed necessary by event staff or emergency personnel. I understand that I am responsible for any costs associated with such treatment.

8. Code of Conduct

I agree to conduct myself respectfully toward staff, volunteers, vendors, and fellow participants. I understand that disruptive, unsafe, or inappropriate behavior may result in my removal from the event without refund.

9. General Agreement

I certify that I am at least 18 years of age and that I have read this waiver and fully understand its terms. I understand that by signing below, I am waiving substantial legal rights, and I do so voluntarily and knowingly.
