**MABOH WELLNESS COLLECTIVE
LIABILITY WAIVER & RELEASE FORM**
By attending Thrive Wellness Sunset Club 2.0 on January 18th, 2025 at Thrive Art District, you understand and agree to the following:
1. Voluntary Participation
I acknowledge that my participation in all activities, including but not limited to yogilates, cold plunge, physical therapy demos, wellness activations, and vendor services, is completely voluntary.
2. Assumption of Risk
I understand that physical activity involves inherent risks, including but not limited to injury, dizziness, dehydration, muscle strain, allergic reactions, or other physical discomfort. I choose to participate at my own risk.
3. Health Acknowledgment
I confirm that I am physically able to participate and have no medical conditions that would prevent my safe involvement in fitness or wellness activities. I agree to listen to my body and modify or stop participation if needed.
4. Cold Plunge Release
I understand the cold plunge may cause increased heart rate, shock-like sensations, or discomfort. I agree to enter and exit at my own discretion and responsibility.
5. Vendor Services
I understand that services provided by vendors (permanent jewelry, flash tattoos, physical therapy demos, food/beverages, supplements, etc.) are optional and I release MABOH Wellness Collective, Thrive Art District, and all partners from liability related to those services.
6. Personal Property
I acknowledge that I am responsible for my personal belongings and valuables. MABOH Wellness Collective and the venue are not liable for lost, stolen, or damaged items.
7. Photography & Media Release
I grant permission to MABOH Wellness Collective and partnering brands to capture photos and video during the event and to use these materials for marketing, social media, and promotional purposes.
8. Release of Liability
By attending, I release MABOH Wellness Collective, Thrive Art District, event staff, instructors, vendors, and partners from any and all liability, claims, or demands arising from my participation. This includes injuries, illness, or damages incurred during or after the event.
9. Agreement
I acknowledge that by attending this event, I have read, understood, and agree to this waiver. My attendance serves as my electronic signature and full acceptance.
