This waiver and release (“Agreement”) is entered into by the undersigned participant (“Participant”) in connection with their participation in the Thrive Wellness Sunset Club (the “Event”) hosted by MABOH WELLNESS COLLECTIVE.
1. Assumption of Risk
I acknowledge and fully understand that participation in activities at the Event—including but not limited to workouts, yogilates, cold plunge, sauna, physical therapy, wellness treatments, vendor activations, food and beverage sampling, and related activities—carries inherent risks of injury, illness, or harm. I voluntarily assume all risks, known or unknown, associated with my participation before, during, and after the Event.
2. Release of Liability
I hereby release, waive, and discharge MABOH WELLNESS COLLECTIVE, its owners, organizers, employees, vendors, contractors, partners, and venue hosts from any and all liability, claims, demands, or causes of action arising out of or related to any loss, injury, damage, or harm sustained by me or my property in connection with the Event.
3. Medical Consent
I affirm that I am physically fit and capable of participating in the activities offered. I assume responsibility for consulting with a physician prior to participation. In the event of a medical emergency, I authorize MABOH WELLNESS COLLECTIVE and its representatives to secure medical treatment deemed necessary, at my sole cost and expense.
4. Personal Property
I understand that I am responsible for my personal belongings at all times. MABOH WELLNESS COLLECTIVE is not liable for any lost, stolen, or damaged property during the Event.
5. Media Release
I grant MABOH WELLNESS COLLECTIVE the right to use photographs, videos, and recordings taken at the Event for promotional and marketing purposes without compensation.
6. Governing Law
This Agreement shall be governed by the laws of the State of Florida.
7. Acknowledgment & Acceptance
By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Agreement. I further acknowledge that I am at least 18 years of age (or have the consent of a parent/guardian if under 18).
