By participating in the Y2K Skate Party hosted by Fitness Ambassadors and SweatPals, I acknowledge and agree to the following:

Assumption of Risk
I understand that roller skating and related activities carry inherent risks of injury, including but not limited to falls, collisions, and overexertion. I voluntarily assume all risks associated with participation in this event.

Release of Liability
I hereby release, waive, and discharge Fitness Ambassadors, SweatPals, event organizers, volunteers, sponsors, and Thunderbird Roller Rink from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in this event.

Medical Acknowledgment
I affirm that I am physically fit to participate in roller skating and group activities and have not been advised otherwise by a qualified medical professional.

Media Release
I grant permission to Fitness Ambassadors and SweatPals to use photographs, videos, and audio recordings taken during the event that may include my image, likeness, or voice for promotional purposes without compensation.

Equipment Responsibility
I understand that skate rental is included, and I agree to return all borrowed equipment in the same condition in which it was received. I am responsible for any damage or loss caused by my misuse of equipment.

Minor Participation
If signing on behalf of a minor, I affirm that I am their parent or legal guardian and consent to their participation under the terms of this waiver.
