Crave Fitness Pilates Class Waiver & Release of Liability. By signing below, I acknowledge and agree to the following:

Voluntary Participation & Assumption of Risk:
I understand that participating in any physical fitness class, including Pilates, involves inherent risks of injury or illness. I voluntarily choose to participate in this class and assume full responsibility for any and all injuries, damages, or losses that may occur, regardless of cause.
Health & Medical Condition:
I affirm that I am in good physical condition and do not suffer from any medical condition that would prevent or limit my participation. I agree to inform the instructor of any relevant physical limitations prior to the start of the class.
Release of Liability:
I hereby release, waive, and discharge Crave Fitness, its instructors, owners, employees, and agents from any and all liability, claims, demands, or causes of action, including those caused by negligence, arising out of or related to my participation in this Pilates class.
Age Requirement:
I confirm that I am 18 years of age or older.
Media Release:
I understand that photos or videos may be taken during the class for promotional or social media purposes. By signing this waiver, I grant permission for Crave Fitness to use my image, likeness, or voice in any such materials without compensation.
