Join us for our monthly Upleveled Walk Club every Second Wednesday of the month in collaboration with The Arizona Sands Club.&#x20;

We will meet outside the University of Arizona football stadium and take a 45 min stroll where you are encouraged to connect with others. We will end back at the Arizona Sands Club with a mindful cool down. \* optional \* All walk club attendees are invited to dine or hang out at the Arizona Sands Club after.&#x20;

UPLEVELED WELLNESS LLC / UPLVL FITNESS EVENTS
Digital Participant Waiver, Release of Liability & Assumption of Risk
Please read carefully. This is a legally binding agreement.
By registering for and participating in any classes, workouts, wellness programming, coaching sessions, or events (“Activities”) organized by Upleveled Wellness LLC / UPLVL Fitness Events (“Organizer”), including outdoor events held at stadiums, universities, or partner venues, you acknowledge and agree to the following:
1. Voluntary Participation & Assumption of Risk
I understand that fitness and wellness activities involve physical exertion and inherent risks including but not limited to injury, illness, falls, collisions, overexertion, equipment use, uneven terrain, heat exposure, dehydration, and risks associated with outdoor and stadium environments. I voluntarily choose to participate and assume all risks.
2. Outdoor & Stadium Environment Risks
I understand Activities may occur in outdoor stadium environments including bleachers, turf, stairs, concrete surfaces, loud music, crowds, and changing weather conditions. I accept full responsibility for my safety when entering, exiting, and participating.
3. Health Responsibility
I confirm that I am physically able to participate and will monitor my own condition. I will modify exercises as needed and stop immediately if I feel pain, dizziness, or distress.
4. No Medical or Mental Health Advice
I understand that Organizer provides fitness, wellness, and mindset coaching for educational purposes only and does not provide medical, psychological, or therapeutic services.
5. Release of Liability
To the fullest extent permitted by Arizona law, I release and hold harmless Upleveled Wellness LLC, UPLVL Fitness Events, its owners, coaches, employees, volunteers, contractors, DJs, sponsors, partners, venues, property owners, universities, and affiliated organizations from any claims or damages arising from participation, including claims based on ordinary negligence.
6. Indemnification
I agree to indemnify and hold harmless the Released Parties from claims resulting from my participation or failure to follow safety instructions.
7. Rules & Conduct
I agree to follow all event instructions and safety guidelines. Organizer may remove participants acting unsafely without refund.
8. Emergency Medical Authorization
I authorize emergency medical care if needed and understand that I am responsible for any related costs.
9. Media Release
I grant permission for photos and videos taken during Activities to be used for marketing and promotional purposes unless I notify Organizer in writing prior to participation.
10. Governing Law
This agreement is governed by the laws of the State of Arizona.
