WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT
THIS WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT (THE "AGREEMENT")
CONTAINS A WAIVER OF YOUR RIGHT TO SUE, A RELEASE OF ALL CLAIMS, AS WELL AS OTHER PROVISIONS
THAT AFFECT YOUR LEGAL RIGHTS. PLEASE READ ALL OF THE PROVISIONS OF THIS AGREEMENT VERY
CAREFULLY.
WAIVER:
In return for being permitted to participate in the event identified below, including all
associated activities and services offered at the event, and use of the premises, facilities,
staff, equipment, and transportation (collectively the "Event"), I, for myself and for my
heirs, representatives, executors, administrators, successors, assigns, and any other person or
entity acting on my behalf or on behalf of my estate, do hereby release, waive, discharge, and
promise not to sue the Released Parties (as ...
This waiver applies only to claims of ordinary negligence and does not release any claims
arising from gross negligence, willful misconduct, or recklessness, to the extent such a waiver
is not permitted under Georgia law.
The term "Released Parties" means WellSocialCo LLC, all sponsors, advertisers, service and
treatment providers and other persons involved in the orchestration and execution of the Event,
the owners and lessors of the premises where the Event is located, and all officers, directors,
employees, partners, and shareholders of any of the foregoing.
DESCRIPTION OF EVENT & ACTIVITIES:
Event activities include, without limitation, the serving and consumption of food and
beverages, cold plunges, ice baths, saunas, compression therapy, stretching, physical activity,
intravenous (IV) treatments, red light therapy, congregation of people, dunk tanks, inflatable
slides and obstacle courses, chiropractic treatment, massage, haircuts, beard trims, workout
classes, padel, and various other activities.
Some services such as IV therapy, chiropractic care, and other wellness treatments may be
provided by independent third parties who are licensed professionals where required by law. I

understand that these service providers operate independently and I agree to review and accept
any applicable treatment-specific disclosures or consents before participating in such
services.
ASSUMPTION OF RISKS:
I acknowledge and agree that participation in the Event is completely voluntary, and that the
Event activities involve certain inherent risks which cannot be completely eliminated
regardless of the care taken to avoid them. These risks may include, without limitation: death,
paralysis, heart attacks, stroke, electrocution, drowning, hypothermia, frostbite, exhaustion
or heat stroke, eye, joint, or bone injuries, concussions, loss of consciousness, sprains,

allergic reactions, burns, exposure to pathogens including COVID-19, and exacerbation of pre-
existing conditions.

By clicking the consent box or electronically signing this Agreement, I confirm that I have
read and understand the risks associated with this event, including serious injury or death,
and I still wish to participate. I represent that I have consulted or knowingly declined to
consult with a medical provider regarding my participation.
INDEMNIFICATION AND HOLD HARMLESS:
I agree to indemnify, defend, and hold the Released Parties harmless from and against any and
all claims, actions, suits, procedures, costs and expenses (including attorney and expert
witness fees and court costs), damages, and liabilities that may arise out of my breach of this
Agreement or participation in the Event.
REPRESENTATION OF AGE:
I hereby represent and warrant that I am at least 21 years of age at the time of signing this
Agreement. I understand and agree that individuals under the age of 21 are strictly prohibited
from participating in or attending the Event. I understand that I may be required to present
valid government-issued identification to confirm eligibility, and that failure to do so may
result in denial of entry without refund.
PUBLICITY RELEASE:

I hereby permit and authorize the Released Parties to: (a) use my name, image, and/or voice
(collectively "Likeness") for any advertising or other lawful purpose; (b) to edit or modify my
Likeness; and (c) assign these rights to others. I waive any right to compensation or royalties
related to use of my Likeness. If I do not wish to appear in photographs or video content, I
will notify event staff at check-in.
Use of my likeness will be limited to event-related promotional, educational, or marketing
content created within 12 months of the Event, and may be used across print, social media, and
digital formats.
SEVERABILITY:
If any portion of this Agreement is held invalid, the remaining portions will continue to have
full legal force and effect.
GOVERNING LAW AND VENUE:
This Agreement shall be governed by and construed in accordance with the laws of the State of
Georgia. The exclusive venue for any disputes arising under this Agreement shall be the state
or federal courts located in Fulton County, Georgia.
CONSENT TO ELECTRONIC SIGNATURE:
I agree that clicking on the consent box or electronically signing this Agreement will have the
same effect as if I were physically signing this Agreement and is evidence of my intent to be
bound by this Agreement.
ACKNOWLEDGMENT OF UNDERSTANDING:
I have read this entire Agreement, understand its terms, and confirm that I am signing
voluntarily without relying on any statements not expressly contained herein.
