I understand that Move to the Beat is a high-intensity interval training (HIIT) class involving strength, cardio, and
rhythm-based exercises. I affirm that I am in good physical condition and able to participate in this class. I understand
that participation carries inherent risks, including but not limited to sprains, muscle strains, broken bones, heat
exhaustion, or other potential injuries.
2. Voluntary Participation
I voluntarily choose to participate in this activity and assume all risks, known or unknown, related to my participation.
3. Medical Disclaimer
This fitness class is not a substitute for medical attention, examination, diagnosis, or treatment. If I experience any pain
or discomfort, I will listen to my body, adjust the movement, and inform the instructor. I understand that it is my
responsibility to consult with a physician prior to and regarding my participation in this class.
4. Photo & Video Release
I grant permission to Move to the Beat and Wear the Damn Shorts, LLC to use photos, videos, or recordings taken
during the class for marketing, promotional, or social media purposes, unless I opt out in writing prior to the event.
5. Release of Liability

I hereby release and hold harmless the instructors, event partners, affiliates, and Wear the Damn Shorts, LLC from any
liability, injury, loss, or damage sustained while participating in the class, including any claims arising from negligence.
COVID-19 & ILLNESS POLICY
I confirm that:
- I am not currently experiencing symptoms of COVID-19 or other contagious illness.
- I have not tested positive for COVID-19 within the past 5 days.
- I will not attend class if I feel unwell or have been in close contact with someone who has tested positive.

By signing below or attending this class, I acknowledge and agree to the following:

I am voluntarily participating in a yoga class or event led by Carissa Parker (yogawithcariss). I understand that yoga involves physical movement, breath work, and stretching, which carry potential risks including, but not limited to, injury, strain, or accident.
	2.	Health Acknowledgment
I confirm that I am physically fit and have no medical condition that would prevent me from safely participating in this activity. I understand it is my responsibility to consult with a physician before beginning any new fitness program.
	3.	Assumption of Risk
I understand that participation in these classes includes inherent risks and I assume full responsibility for any injury or damage, known or unknown, that I may incur as a result of participating in these sessions, including any outdoor, indoor, or off-site locations.
	4.	Release of Liability
In consideration for being permitted to participate in this class, I, for myself, my heirs, personal representatives or assigns, release and discharge Carissa Parker, yogawithcariss, or the event location or venue, and any affiliates from any claims, demands, or causes of action arising from any injury or damages that may occur as a result of my participation.

I understand that photos or videos may be taken during class and used for promotional purposes. If I prefer not to be photographed, I will inform the instructor before class begins.
	6.	Weather & Refund Policy
For outdoor classes, I understand that weather conditions may cause class changes or cancellations. I agree to check communications from the instructor and understand that class fees are non-refundable but may be credited to a future class at the instructor’s discretion.
