I acknowledge and agree that there are inherent risks associated with exercise, and I willingly agree to hold only myself responsible for my personal safety, for my personal property, and for accidents which I may cause or which may befall me and any and all bodily injury, should it occur whether inside the facility, or in the shopping and parking area adjacent to Corazon Latino Dance Studio.

By legally assuming responsibility for my well-being, my personal property and any accidents in which I am involved, I relieve Corazon Latino Dance Studio, Corazon Latino's owners, staff and clients, and the building's owner, Starpoint Property Management Company, from any and all liability, should any calamity occur.

I freely state that I am participating of my own free will in Corazon Latino's aerobic dance classes, and in other activities, classes and events they offer now or those which they may offer in the future. I further state that I am in good health and am sufficiently physically fit to participate in any of Corazon Latino's classes, activities or events which I choose to attend and those in which I choose to participate.
