PARTICIPANT WAIVER & RELEASE OF LIABILITY

PLEASE READ CAREFULLY BEFORE SIGNING

I, the undersigned participant, acknowledge and agree to the following:

1. Voluntary Participation & Assumption of Risk

I understand that participation in events hosted or organized by Authentic Curated Experiences (ACE) may include wellness, movement, educational, social, recreational, or experiential activities. These events may occur indoors or outdoors and may involve physical activity, environmental conditions, group interaction, or other elements that carry inherent risks.

I understand that risks may include, but are not limited to, physical exertion, slips or falls, environmental factors, weather, heat or cold exposure, dehydration, contact with other participants, use of facilities or equipment, and other known or unknown hazards.

I knowingly and voluntarily assume all risks, foreseeable or unforeseeable, related to my participation, whether occurring before, during, or after the event, and whether on or off the event premises.

2. Health & Personal Responsibility

I certify that I am physically and mentally capable of participating and that I assume full responsibility for monitoring my own condition and limitations. I understand it is my responsibility to consult a qualified healthcare professional prior to participation if I have any health concerns or conditions that may affect my ability to safely participate.

3. No Medical Advice or Physician–Patient Relationship

I understand that ACE events are not medical care, medical advice, diagnosis, or treatment. Participation does not create a physician-patient, provider-patient, or therapeutic relationship with ACE, its founders, hosts, speakers, vendors, or affiliates.

Any information shared is for general wellness, educational, or social purposes only and should not be relied upon as medical advice.

4. Release of Liability & Indemnification

In consideration for being permitted to participate, I hereby release, waive, discharge, and hold harmless Authentic Curated Experiences (ACE), its founders, organizers, employees, volunteers, contractors, partners, sponsors, venue owners, and affiliates from any and all claims, demands, causes of action, damages, or liabilities of any kind, including those arising from ordinary negligence, to the fullest extent permitted by law.

I further agree to indemnify and defend ACE against any claims brought by me or on my behalf related to my participation.

5. Emergency Medical Authorization

In the event of an emergency, I authorize ACE and its representatives to seek emergency medical care on my behalf if deemed reasonably necessary. I understand that I am solely responsible for any medical or related costs incurred.

6. Photography, Video & Media Release

I grant permission for ACE to photograph, record, or otherwise capture my likeness during events and to use such media for marketing, promotional, educational, or business purposes without compensation or additional approval.

7. Conduct, Guidelines & Self-Direction

I agree to follow all event guidelines and venue rules. I understand that participation is self-directed, and that ACE does not provide individualized supervision, instruction, or safety monitoring unless explicitly stated.

8. Alcohol & Substances (If Applicable)

If alcohol or other substances are present at an event, I acknowledge that any consumption is voluntary and that ACE does not provide medical oversight, encouragement, or supervision of use.

9. Governing Law

This agreement shall be governed by and interpreted in accordance with the laws of the State of Arizona.

10. Binding Agreement & Electronic Signature

I understand that this waiver is a legally binding agreement. By signing below, I acknowledge that I have read and understood this document and voluntarily waive certain legal rights. I agree that this waiver may be executed electronically and that such signature has the same legal effect as a handwritten signature.
